Diagnosis of sciatica 📌

👉 Sciatica is largely a clinical diagnosis based on the person’s symptoms and findings on examination. A history of leg pain worse than back pain or pain below the knee should raise suspicion of sciatica. Inquire about the onset and distribution of pain, and associated symptoms such as tingling sensation, numbness, or muscle weakness in the legs. https://pubmed.ncbi.nlm.nih.gov/31744805/

Cauda Equina Syndrome (CES) 🤓
📍 The spinal cord is the continuation of the medulla oblongata, which ends at the medullary cone. A bundle of spinal nerves and spinal nerve roots begins from the medullary cone and forms the cauda equina. Specifically, the cauda equina consists of the second through fifth lumbar nerve pairs, all of the five sacral nerve pairs and the coccygeal nerve. https://pubmed.ncbi.nlm.nih.gov/19104682/, https://pubmed.ncbi.nlm.nih.gov/15202874/
👉 One of the most common disorders of the cauda equina is presented as CES. 👉 The main cause of CES is the lumbar is a central disk herniation (LDH, s. figure ), mostly at L4/5 or L5/S1 level. 👉 The early stage of CES is often referred to as cauda equina syndrome incomplete (CES-I). The patient may have reduced bladder or urethral sensation and/or saddle sensory disturbance, but they retain bladder control. 💡 CES-I can evolve in CES-R. CES-I is presented in 30-50%, CES-R is presented in 50-70% of cases. https://pubmed.ncbi.nlm.nih.gov/21193933. The outcome of CES-I is generally favorable if there is an urgent surgical decompression within 48 hours.
