Note: Pinaka matagal na proseso ang pagkuha ng medical certificate, ayon sa client ko - isang linggo nila itong prinoseso. Kumpara sa ibang documents na pwede namang ma proseso sa loob ng isang araw.

NOTE: Sa client ko, nakuha nila ang medical certificate nila after 1 week dahil may bayad.
Pero kapag libre, dadaan ka muna sa 14 days quarantine

2. Pag nakakuha kana ng medical certificate, ay pwde kanang bumili ng ticket mo pauwi, kasi may fifill-upan ka po na Form sa barangay nyo para sa pagkuha ng Travel Pass. Kailangan po kasi sa travel details nyo.

***At pag nakuha mo na lahat ng nasa 1 and 2,

3. Pumunta sa PNP para iprocess ang TRAVEL AUTHORITY
-maghintay lamang ng limang (5) araw para marelease at matanggap mo ang iyong TRAVEL AUTHORITY

Paalala:
•Habang magpo process ng medical certificate, makipag- coordinate po muna sa LGU na uuwian ninyo.
•Tawagan ang kamag-anak at magpakuha ng ACCEPTANCE LETTER.
•Tatawagan kasi ng LGU kung saan ka ngayon ang uuwian mong probinsya o LGU para kumpirmahin ang iyong pag-uwi.
•Mas madali ang pagkuha ng travel authority kapag ang iyong kamag-anak sa iyong uuwian ay nakapunta rin sa LGU municipal office para ipaalam na uuwi ka at makakuha ng acceptance letter.
•Pag nakakuha na ng ACCEPTANCE LETTER ang iyong kamag-anak ay pakuhanan ito ng picture at nararapat na isend ng kamag-anak mo sa messenger dahil ikaw na byahero ay ipapakita mo ito sa Pnp upang di kana mahirapan sa pagkuha ng travel authority.

2. Certificate of Acceptance from LGU

✈️DAVAO✈️
1. Negative RT-PCR (swab) test results taken within 24 to 72 hours from departure date. This is not mandatory upon check-in, but shall be required upon exit at the Davao International Airport.
2. Guests who tested in their place of origin within 24-72 hours before departure, but have not received their results, will not be allowed to exit the Davao Airport, until the test result is available.
3. Children 12 years old and below are exempted from the RT-PCR requirement upon check-in, as long as the accompanying adult complies with the requirement.

✈️DUMAGUETE✈️
For Locally Stranded Individuals (LSI), the requirements are:
1. Medical Certificate and Travel Authority, or Travel Pass from the Philippine National Police. LSIs are subject to local health and safety protocols of the receiving LGUs
For Authorized Persons Outside Residence (APOR), the requirements are:
1. Medical Certificate and Mission Order, as well as coordination with the provincial IATF-EID. For contact information of your local tourism offices.

✈️GENERAL SANTOS✈️
The City of General Santos requires guests to present a Travel Authority, as well as a negative RT-PCR (swab) test result taken within 72 hours before departure date.

✈️ILOILO✈️
For Local Stranded Individuals (LSI) and Returning Overseas Filipinos (ROF):
1. Negative RT-PCR (swab) test result valid for three (3) days before departure date
2. Medical Clearance certification issued by the City/Municipal Health Office
3. Travel Authority issued by Chief of Police of the City/Municipality of origin
4. Any Valid Government ID (showing relevant information such as residence, citizenship or occupation)
5. Notice of Coordination from LGU (Proof of coordination)

✈️KALIBO✈️
For Locally Stranded Individuals (LSI):
1. Travel Authority issued by Joint Task force COVID Shield
2. Negative RT-PCR (swab) test result issued by DOH-Accredited testing facility

1. Entry Authorization Pass from the PDRRMO
2. Letter request or travel order and government-issued ID to PDRRMO
3. Medical certificate or certification issued by the City Health Officer/ Municipal Health Officer or DOH-accredited hospitals that guest has not been in contact with, nor a suspect/probable/confirmed COVID-19 case
4. May be subject to RDT rapid test upon payment and the health authorities of the province shall determine whether said APOR shall be admitted to a facility-based quarantine

✈️PUERTO PRINCESA✈️
For Locally Stranded Individuals (LSI):
1. Medical Certificate issued by Local Health Office
2. Travel Authority or Travel Pass issued by Local Police
3. Proof of Residence copy (with complete city or municipal address)
For Returning Overseas Filipinos (ROF):
1. Travel Authority or Travel Pass
2. Negative RT-PCR (swab) test result and Quarantine certificate issued by Bureau of Quarantine
3. Proof of Residence copy (with complete address)
4. Valid Passport
For Authorized Persons Outside Residence (APOR)
1. Company ID or Employee Certificate

✈️TACLOBAN✈️
For guests flying to Tacloban, the City of Tacloban requires all Locally Stranded Individual (LSI) guests to present a Medical Certificate and Travel Authority to be accepted for flight.

✈️ZAMBOANGA✈️
1.Travel Authority issued by JTF-CV SHIELD/li>
2. Medical Clearance from local health office (CHO/MHO and PHO) stating that they have no flu -like symptoms
3. Negative RT-PCR (swab) test result. if none, guest will be subjected to a facility-based.
4. Prior coordination with the City Tourism Office for the availability of the quarantine facility.

TRAVEL REQUIREMENTS FOR LSIs AND PRIVATE APORs ARRIVING FROM MANILA AND RESIDING IN CEBU CITY

Following the mandate of the City Government of Cebu, starting 28 August 2020, all domestic passengers classified as locally stranded individuals (LSIs) and private Authorized Persons Outside of Residence (APORs) arriving from Manila and residing in Cebu City are required to submit a Letter of Coordination issued by their barangay of residence in Cebu City prior to their arrival at Mactan-Cebu International Airport (MCIA). This is in addition to the Travel Authority issued by PNP JTF COVID Shield for LSIs.

Upon arrival in Cebu City, all LSIs and private APORs must:
- Undergo a 14-day quarantine at a facility determined by the Barangay of Residence in Cebu City, and;
- Undergo mandatory RT-PCR swab test on the 3rd to 5th day from their arrival date in Cebu City. The cost of swab test is shouldered by the City Government of Cebu.

To request for the Letter of Coordination, send a request via email to City Government of Cebu at [email protected] or contact the hotline numbers at 0945 278 5076 (Globe) or 0961 523 6009 (Smart). Passengers can also contact their respective barangays via this linked directory (http://bit.ly/CebuCityBarangayDirectory).

Passengers are strongly advised to continuously monitor the status of their flights and to ensure that they have the necessary documents in compliance with the regulations and guidelines of the LGU of their point of origin and destination.
