Care coordination after diagnosis is so important. For so many reasons.
Coordination among providers is essential to ensure that patients receive the right care at the right time.
Tailored treatment plans that address each individual's specific needs and goals.
Continuity of care: patients often require ongoing care and support. Coordination among providers helps to ensure that care is consistent and coordinated over time.
Family involvement: Families play a critical role in the care of children with developmental behavioral concerns. Coordinated care helps to ensure that families are involved in decision-making and that they have the support and resources they need to care for their loved ones.
Coralis Health is here to help you navigate through it all.
