How would you chart this rate change? Would you make a blank note? Would you chart is as a reassessment under cardiac tab? What is you standard or practice.
How do you know what you know? If asked, how do you know the standard of care for say a STEMI or septic protocol? Is it just because your educator says so, because that won’t hold up if asked during a legal proceeding. Where does the info actually come from? Look up references in the cited research and understand the support behind the research, this is how you defend your practice!
