Looking for an LPN or RN to fill Clinical Care Coordinator position.

Position is located primarily in Cookeville, TN, but does require travel to various physician medical offices across the Upper Cumberland to assist with quality measure collection in an effort to improve healthcare in our region. Individual will report data using various portals to deliver value to our patients and practices.

Required LPN or RN with clinical experience, preferably in a physician office setting, with working knowledge of EMR system(s). Good organizational and communication skills are important, Excel experience preferred, and must work well with staff and clients. Must have valid driver's license.

CCHI CEO, Brent D. Staton, M.D. is a 50 year-old family medicine physician in rural Tennessee and was previously in excellent health. He placed 6th in a triathlon two years ago. Pulmonary function tests showed that he had the lung capacity of someone in their 30s. His diet was based on healthy foods, and he exercised daily.

He contracted earlier this month. Now his lung capacity is that of a 74-year old. Many in his community still refuse to wear masks to this day, and no state in the country is more infectious than Tennessee right now when it comes to COVID-19.

Cumberland Center for Healthcare Innovation (CCHI), has once again improved care for 14,874 Medicare beneficiaries throughout the state of Tennessee, succeeding in both performance and quality in 2019, according to recently released performance data from the Centers for Medicare & Medicaid Services.

CCHI is a physician-led Accountable Care Organization (ACO) uniquely comprised of Family Medicine and Primary Care Providers in a rural setting. CCHI consists of 126 total providers serving CMS, Medicare Advantage, and commercial patients residing in 77 of the 95 counties in Tennessee.

As independent physicians, our members came together voluntarily to deliver coordinated, high-quality care for their patients. The goal of coordinated care is to ensure that patients, especially the chronically ill, get the right care at the right time, while avoiding unnecessary duplication of services and preventing medical errors.

ACOs empower local physicians, hospitals, and other providers to work together and take responsibility for improving quality, enhancing patient experience, and keeping care affordable. The Medicare Shared Savings Program (MSSP) creates incentives for ACOs to invest in transforming care. When an ACO exceeds quality and financial thresholds-demonstrating achievement in high-quality care and wiser spending of healthcare dollars- it is able to share in the savings generated for Medicare. CCHI has achieved quality scores of between 87.24% in Year 1 to a high of 98.48%.

In 2019, CCHI generated $13,221,810 of gross savings to Medicare. Since the organization’s start in 2012, CCHI has achieved Savings seven of seven years and saved CMS a total of $43,272,667.

CCHI continues to demonstrate the value of rural independent primary care physicians in our communities, state, and nation. Our communities and state benefit through healthier families, allowing for a more productive workforce and more opportunity for rural development. Delivery of exceptional healthcare, including timely follow-up appointments after discharge, wellness visits, and closure of additional care gaps, does make a difference.

CCHI providers are proving that high quality medical care is indeed possible in rural Tennessee and rural America. CCHI Chief Executive Officer, Brent Staton, MD, and Chief Medical Officer, Ty Webb, MD, FAAFP, congratulate all ACO members and staff for this achievement.

The opportunity for continued success remains great. The future of our ACO is bright. We achieved Shared Savings.
