The ICD-10-CM code set update for fiscal year (FY) 2024, effective Oct. 1, 2023, adds 395 new codes, deletes 25 existing codes, and revises 22 codes (nine of which are to the short descriptor only). 2Q2023 Coding Clinic: D/T=W/?

The second quarter’s Coding Clinic has been released! It’s always exciting and a bit anxiety provoking to read through the Clinic to see what’s new, what’s clarified, and what’s been corrected. This month we’ll review 4 of the more provocative topics presented for the second quarter of 2023. For the first week in the series, we’re reviewing this equation: D/T=W/? We strongly encourage you to review the Coding Clinic for a more complete review/discussion of the topics.

Anemia d/t Chemotherapy – Although the title of the coding clinic reads “Anemia d/t Chemotherapy,” the question (and advice) expands the discussion to a review of the Official Guidelines and nonessential modifiers. The question from the coding professional seeks advice concerning the assumption of a causal relationship between anemia, chemo, blood loss, or other potential underlying cause based on the subterm “due to,” even if the physician doesn’t link the relationship in the documentation. For example: would we apply the “With” Guideline when the indexed diagnosis includes “with” as a nonessential modifier, as in Anemia due to (in) (with) Antineoplastic Chemotherapy, D64.81?

Coding Clinic responds with a resounding “No” stating the “With” Guideline (see below) doesn’t apply here because the classification doesn’t presume a relationship between the two conditions. Further, Coding Clinic explains the terms “with” and “due to” are not interchangeable; the physician must provide supporting documentation to link the two conditions. So, D/T≠W/!

Official Guidelines for Coding and Reporting>Conventions>15. “With”
“…For conditions not specifically linked by these relational terms in the classification or when a guideline requires that a linkage between two conditions be explicitly documented, provider documentation must link the conditions in order to code them as related.”

“The word “with” in the Alphabetic Index is sequenced immediately following the main term or subterm, not in alphabetical order.”
