Classification of aortic dissection

Several different classification systems have been used to describe aortic dissections. One such classification is based on chronicity and labels aortic dissections as hyperacute (30 days). The systems commonly in use are based on either the anatomy of the dissection or the duration of onset of symptoms prior to presentation. The Stanford system is used more commonly now, as it is more attuned to the management of the patient.

DeBakey:
The DeBakey system, named after cardiothoracic surgeon Michael E. DeBakey, is an anatomical description of the aortic dissection. It categorizes the dissection based on where the original intimal tear is located and the extent of the dissection (localized to either the ascending aorta or descending aorta or involves both the ascending and descending aorta.

Stanford:
The Stanford classification is divided into two groups, A and B, depending on whether the ascending aorta is involved.

Total Artifical Heart.

A French company Carmat has received approval in Europe for their device which is a total artificial heart.

This is a fully artificial mechanical heart that is designed for long term use up to 5 years. Previous devices like this were only utilized short term as a bridge to transplant, however this one can also be used as "destination therapy"

We already have devices called LVADs (left ventricular assist devices) however those can only be used for left ventricular failure. Biventricular failure has limits options. This fancy thing may buy extra time for folks who either cannot get a donor heart or are on a long waiting list for one.

This device weighs 900g (about 3x a normal human heart.

The only other artificial heart currently approved in the US and Europe is Syncardia. Advantages of the Carmat include no large external components as its all implantable (syncardia requires a backpack to wear), and there is biological material of the valves - so the risks of clot is reduced and anti coagulation may not be required at all! That is huge.
