- TWO CANALLED PREMOLAR
This was a referred case where the previous practitioner could find only one canal. The second canal was located distally. These cases are tricky to obturate because there isn't enough space to place two gutta percha cones together and condensation of one cone results in blockage of the other. I usually downpack one canal, then remove any inadvertent blockage from the other using a micro-opener before downpacking the other canal.
Our State of the art microscope gives an extreme closeup view of the tooth so that the root canal can be done with maximum accuracy. It reduces the chance of missing anything that is not visible to the naked eye. The Assistoscope ensures that the dental assistant has the same view as the doctor which enhances accurate teamwork.
You will experience mild discomfort in the tooth for a few days and will not be able to use that tooth for chewing. The pain felt is usually very mild and is easily controlled by the medication prescribed for you.
- SPOT THE GUTTA
This patient insisted that the previous dentist had shown her pictures of all 3 canals filled. Once we opened the tooth, we found out where the missing gutta percha was :-).
No. The primary aim of the Root Canal Treatment is to relieve dental pain, not add to it. The perception of RCTs being painful was due to the lack of proper tools and anaesthesia in the 1800s. With the latest technologies, anaesthetics and microscopes, they feel more or less like having a filling placed.
- Deep split
Upper premolars with a single canal that bifurcates apically can be a challenge to clean, shape and obturate. A microscope helps in eliminating guesswork when dealing with clinical scenarios like these.
All dentists, including your general dentist, received some training in root canal treatment while in dental school. But endodontists (root canal specialists) spend three more years studying endodontic procedures in more meticulous detail.
At the end of three years, they are specially trained to handle complex endodontic procedures. Many dentists refer their root canal cases to a specialist endodontist.
: MISSED MB2 RE-TREATMENT
The mb2 is probably the most missed canal in clinical endodontics.
We prefer selective re-treatment in these cases if the other canals seem well filled.
The surgical microscope helps us in delivering the most conservative treatment and preserving the existing crown.

Here is a calcified incisor where the previous clinician perforated palatally in an attempt to locate the canal. Early identification of calcified canals and prompt referral can help prevent extensive (and often irreversible) damage to the tooth structure.

RCT on teeth with a deep pulp chamber are highly prone for iatrogenic errors. These are best referred to a specialist using a microscope. Proper magnification and visualization ensures the most precise and conservative treatment.

Re-treating failed root canals are a routine procedure that we do in our clinic. We do every step under the microscope which enables us to precisely remove previously placed posts, restorative material and gutta percha. It also enables us to locate and treat missed anatomy.

: CONSERVATIVE ACCESS TO PRESERVE EXISTING INDIRECT RESTORATION

Case Report - Selective re-treatment of missed mb2 canal.
This patient had a sinus tract in relation to a previously root canal treated and crowned maxillary first molar.
The previous clinician had placed fiber-posts in both mb and palatal canals. Disassembly of the whole tooth would not be the most conservative option. Selective treatment of only the missed mb2 canal gives this tooth the best chance for survival.
