All hospital deaths reviewed by the NSW state coroner since 2012 had one or more of the following issues highlighted in court.

1. Vital signs not taken/not taken according to policy
2. Abnormal vital signs ignored or not escalated as per policy
3. Family/Patient concerns downplayed

Remember to take observations, escalate care and tell patients and their relatives how they can pass on concerns to staff.
