tip of the day: Finding a scapegoat and pointing fingers at who caused the issue will not prevent it from happening again and will scare others from reporting future issues or near-misses.

Ultimately, it doesn’t matter WHO caused the issue so much as it matters WHAT the issue was and what needs to be put in place to ensure it does not happen again. No blame needs to be a feature of any RCA program implementation.

Root Cause Analysis tip: There should be a very strong correlation between the significance of a problem and its name. They are almost synonymous. To help you get there, challenge your group to look beyond the immediate problem (for example, a seized bearing) and get them to articulate the significance of that seizure. https://www.armsreliability.com/page/resources/blog/defining-your-problem-whats-in-a-name

One of the most important things you can do to get ahead of risk is to identify and understand your potential points of failure and their likelihoods. That’s not to suggest it’s an easy task.

In large asset-intensive organizations, risk can come from many areas. It can be difficult to uncover and even more difficult to quantify. However, with an Integrated APM process, you can better prioritize your resources and get to a place where you can stop fighting fires and start to act on opportunity. How to Make Integrated APM a Reality and Get the Most Out of Your Assets. Traditional approaches to APM have been far too technology centric, with attempts to get value from the installation of modular software applications without the required process, education, support, and plan to generate the supporting content, to be used within the system.

To be successful, Integrated APM must connect both the technology and processes supporting asset management. Read the full blog at https://www.armsreliability.com/page/resources/blog/how-to-make-integrated-apm-a-reality-and-get-the-most-out-of-your-assets

You want the most cost-effective way to do the maintenance build project and deliver optimal asset performance once the project goes into operations, but if you chose to follow a traditional maintenance build path by excluding a data-driven bottom-up analysis, chances are you will not deliver on performance targets.

When you follow an Asset Strategy Management (ASM) process, it’s faster, easier and more cost-effective to develop intelligent plant design and reliability strategies that support your assets through the whole life cycle. Root Cause Analysis tip of the day: Picking and implementing an effective RCA methodology for the entire organization is critical to overcoming the problems of an ad hoc approach. To do this, an RCA champion should be appointed who is responsible for selecting a single RCA tool and then bringing all employees up to speed on how to use the tool. This champion is responsible for the performance of RCA. To keep them accountable, it should become part of their KPIs.
