As National Pain Awareness Week in Canada continues, I would like to briefly discuss the concept of supported self-management, something that is still not well understood in much of healthcare.

What this isn’t is a dismissal of the person living with pain and being told there is nothing more healthcare can do for them, and they are all on their own. Unfortunately, for healthcare providers who do not understand what supported self-management is, that can be the message that is all too often received.

With that, what is supported self-management? The National Health Service of the United Kingdom describes it as “increasing the knowledge, skills and confidence a person has in managing their own health and care by putting in place interventions such as: peer support, self-management education and health coaching”.[1]

Effective self-management is an equitable and collaborative relationship between the person living with pain and their healthcare provider. The goal is to offer the person living with pain education and knowledge on how they can best help themselves, and this is achieved through education and training resources. Supported self-management needs to consider the person within the totality of their life, value them as equal and active partners, offer support in finding and developing solutions, focus on goals, as well as prove access to cincinnal support if and as needed.

There is evidence from the United Kingdom of the positive effects of support self-management in that one study found there were 19% fewer family physician appointments and 38% fewer visits to emergency departments. [2]

Until and unless there is a cure discovered for persistent pain, self-management is foundational to living better despite the pain. However, it does not mean that there is no role for healthcare providers to support the person. This support may involve medically appropriate biomedical interventions, psychological support, nutritional counselling, and more.

Supported self-management is not a matter of healthcare system support or personal pain management rather is healthcare system support and personal pain management, which is a true synergistic relationship.

I’d like to take a moment to recognize two people that I consider leaders in this space: Pete Moore, the founder of the very effective Pain Toolkit (a leading self-management resource in use internationally) and Dr. Owen Williamson who introduced me to the broader concept of supported self-management. I am honoured and privileged to call both friend.

Supported self-management is integral to A Path Forward.

Wow, what a story

41-year-old trashman Bob, who had been married to his wife Sarah for 9 years, ended up raising their twin daughters alone one day. Sarah left them, revealing that the twins were born to another man with whom she had an affair two years after marrying Bob.

Bob battled against all odds to make his daughters happy. He believed they were his world, knowing little there was going to be a new addition soon.

One day, Bob found an abandoned blind baby crying near one of the dumpsters he picked garbage from. "If I can raise two daughters by myself, I can certainly raise him too," he thought and later adopted the poor boy.

The caring father nurtured his son and later discovered the boy had a special interest. Little did he know that 23 years later it would make them millionaires.

“There is no reason for him to be challenging Jorginho like this.

“Look how [far] away he is. The ball has gone. His arm doesn’t need to be up.

“I can’t understand why that isn’t a yellow card and they aren’t looking at that. He is trying to hurt him there.

“You know something that could easily be given as a red card.

"What’s he doing? His intention is to hurt him. It’s not like it’s a challenge and your arm has flailed.

"The ball has gone, he has no reason to do that. None!

“He is lucky that he hasn’t caught him in the temple or something. It’s a bad challenge.”

November 5th to 11th, 2023 is National Pain Awareness Week in Canada, a time to raise awareness of chronic pain and its impacts. While the primary focus must be on those living with pain we can't overlook the economic impact. The direct and indirect cost in Canada is 40.3 billion dollars per year.

Implementing Health Canada's Action Plan for Pain in Canada can reduce the human and financial toll of pain. Track its progress at PainCanada.ca/actionplan.

Raising awareness is A Path Forward.

My story is very typical of that of a pain patient. I was, like many pain patients looking for that magic bullet to take away my pain. Managing pain was like playing a game of snakes and ladders – a game of luck. Most days melted into the next and he became a very poor pain self-manager. He did not exercise or generally look after his body and when something went wrong with it, he expected the doctor to fix him.

My Turning Point!

In July 1996 I attended the INPUT Pain Management Programme (PMP) London. It was described to me as a programme that could help me to increase my confidence and mobility and provide him with life-long skills to self-manage his pain.
