Anterior Cruciate Ligament (ACL) post-surgery presentation by Clare Ardern. ACL outcomes post-surgery:
1. 1/3 return at pre-injury level after 1 year
2. 2/3 RTP at some point post-op
3. 1/3 actively playing sport
In a different study surgery vs conventional treatment was compared. A surgeon is likely to recommend ACL reconstruction so it is important to evaluate the benefits and negatives carefully. This is because at 2 years everyone in the study rated their knee function the same whether they had surgery or not and RTP rates were also similar. Definitely, something to think about and share with your clients as it may help them make a more informed decision about their options!

If your not clued up on concussion and head injuries check out this short video. A few key points to take away:
1. Don't forget your ABCDE during the initial assessement (Airways, Breathing, Circulation, Disability, Exposure).
2. Unconciousneess only actually occurs in about 10% of concussions.
3. The players, medical staff and coaches all have a responsibility to work as a team to identify and moniter possible concussions as they are not always noticed.
Alfredsons eccentric training protocol is currently the gold standard treatment for Achillies Tendonopathy however 180 eccentric heal drops is time consuming and boring. There is recent literature that suggests performing a 6-week do-as-tolerated program (do what is reasonably achievable) of eccentric heel-drop exercises, compared to the recommended 180 repetitions per day may be just as effective. This evidience was based on both VISA-A and VAS scoresIn the future this research needs to be replicated with more participants to ensure its validility and reliablilty.
If the rehabilitaiton programme is reduced and made quicker to complete then this may also help to increase clients adherance to the programme and in turn should speed up the recover process.
Effectiveness of the Alfredson Protocol Compared With a Lower Repetition-Volume Protocol for Midportion Achilles Tendinopathy: A Randomized Controlled Trial | Journal of Orthopaedic & Sports Physical Therapy Abstract Study Design Randomized clinical trial. Objectives To compare the effectiveness of the Alfredson eccentric heel-drop protocol with a “do-as-tolerated” protocol for nonathletic individuals with midportion Achilles tendinopathy. Hydrotherapy is a great way to keep up cardiovascular fitness whilst injured and unable to fully weight bear. Partial weight bearing also reduces the impact on bones, joints, and muscles. If the water is cold it may cause decreases in local edema, nerve conduction velocity (NCV), muscle spasm, and increase in local anesthetic effects (1).
