The Agency for Health Care Administration (Agency) is working to address concerns surrounding the delivery of OT, PT, and SLP therapy services under Telemedicine Rule 59G-1.057. Florida Medicaid intends to cover telemedicine in accordance with guidance from the American Medical Association (AMA) and Centers for Medicare & Medicaid Services (CMS). As indicated in the May 4, 2023 Provider Alert, Florida Medicaid will no longer cover audio-only telehealth services.
Florida Medicaid currently reimburses for telehealth visits at the same rate as face-to-face visits in accordance with current promulgated Florida Medicaid Fee Schedules; both the GT modifier and POS 02 must be appended to the claim. Providers should reference AMA CPT guidelines for the list of applicable therapy codes that are reimbursable via telemedicine.
Please note: health plans have the flexibility to cover services above and beyond the Agency’s fee schedules and coverage policies; however, health plans cannot be more restrictive in coverage than the Agency and promulgated rule, as detailed in their contract. Providers should review their contract with the managed care plans for questions regarding health plan coverage of audio-only telehealth services. Stepping Up on Failure to Cover Mandated Medicaid Speech-Language-Hearing Services for Children ASHA members can help foster states’ compliance with a new law that requires increased federal oversight of how well state Medicaid programs and their managed care organizations discover and treat children’s health conditions.
Regarding Medicaid Telehealth, I was hoping we would see an AHCA alert this past week about telehealth, but they seem to still be working on it so here is what I understand is the current status:
What we Know:
Telehealth is allowed by statute.
All MMA plans should still be paying for telehealth.
AHCA by some oversight did not have their straight Medicaid system set to pay the GT or 59 modifier or the 02 POS so in 2019 the Alliance requested that they address and fix this with their vendor Gainwell. We were working through providing them what they needed to complete the system edit in early 2020 and thought they were pushing the final changes through but then COVID flexibilities hit, and I guess instead of finalizing the system edits they just moved on and forgot about it.
The Alliance and the FLASHA Medicaid Committee have been working with AHCA to get this remedied and had to provide supporting documentation to AHCA multiple times related to telehealth for our professions.
AHCA says they have everything they need for the update to straight Medicaid and are working on getting the change made so that the claims will pay.
It is unclear WHEN they will have it working.
AHCA does not feel it is necessary to include a statement about telehealth being allowed in any of the coverage policies (SLP, PT, OT, EIS) because it is AHCA’s position that telehealth is addressed in the statute.

In order to enhance service delivery for our children and families, we are now extending the opportunity to include therapy assistants. This language is already included in your new Provider Agreement. Assistants may be reimbursed up to $57.60 per session, depending on the discipline and length of session. They are also eligible for travel reimbursement and NESF in defined zip codes, just like therapists.

I just want to let you know that we identified that some recent claims for PT/ST/OT have denied for no authorization. Please tell your association members that we identified the cause of the denials and we were able to fix it on 3/3/23. No action is required of them as we are reprocessing all the impacted claims with a target completion date of 3/17/23. So they will start receiving the payments later this week into next week.
