Two forms that you need to fill in when you confirm your pregnancy:
1. MATERNITY EXEMPTION
2. MATERNITY CERTIFICATE
MATERNITY EXEMPTION card entitles you to free NHS dental care and free prescriptions when you’re pregnant, or have just had a baby. You can get your card by filling in an application form from your doctor or midwife. They will sign and send it for you and you will receive your card in the post. The free NHS treatment covered includes what is clinically necessary to protect and maintaint good oral health. It does not include cosmetic treatments such as whitening, implant, veneers, braces etc as these are only available privately.
MATERNITY CERTIFICATE (MAT B1 form) is a form from the government that gives your employer medical evidence of your pregnancy and when your baby is due. You can request it from your midwife at the antenatal appointment that takes place after your 20-week scan. You'll need a MAT B1 form in order to claim for maternity pay and benefits, whether you're unemployed, self employed, full time or part time.

What is the difference between Private and NHS dental treatment?
The common assumption is that private dentistry is more expensive but this may not always be the case and usually the service and treatment outcome you receive is better and unrestricted.
PRIVATE DENTAL TREATMENT:
> Private dental treatment is there to provide the patient with best possible functional and also cosmetic result using materials and treatments which aim to produce the best appearance. Cosmetic treatment is not available on the NHS.
> You are able to book appoinments at anytime the practice is open including evening and weekends (Saturdays and in some cases Sundays)
> You are able to have the very highest standard of treatment and high quality materials.
> Majority of private practices offer interest free plans in order to help with financial costs of treatment.
> Private dental work such as dentures, crowns, bridges and implants are sent to private laboratories where there is high quality work made and usually more flexibility in returning items.
> Appointments are longer and at a more relaxed pace
> All treatments are available unlike on the NHS
> You are able to have Teeth Whitening done unlike on the NHS
> With private treatments you can request your treatment to be performed by a specialist (private sector provides quick and easy access unlike NHS where there are long waiting lists)
Orthodontic treatments are not available under NHS at the age over 18. Invisalign not offered on NHS.
NHS DENTAL TREATMENT:

> Most NHS practices have specific time slots that you have to meet which is not always ideal for working people or parents. The appointment availability may be limited.
> NHS cleaning is very basic and covers only what is clinically necessary and not specifically remove all stains or for cosmetic reasons.
> The waiting lists or referral times take long time to get arranged or booked and you are not always guaranteed to get accepted
> Treatments not available under NHS are: cosmetic treatments/implants/tooth whitening, hygienists, specialist referrals.
