(Rb) is a common eye cancer caused by mutations in the retinoblastoma gene (RB1). It occurs in about 1 in 17,000 live births, with approximately 8,000 new cases diagnosed worldwide each year. Rb can be hereditary (30–40% of cases) or sporadic (60–70%), resulting from inactivation of both alleles of the RB1 gene, leading to defective Rb protein (pRb) and uncontrolled cell growth.

Clinical features include signs like leukocoria (white eye reflection) and (crossed eyes). is often the first sign, but diagnosis is usually made late, affecting the prognosis. Treatment options for Rb include enucleation, chemotherapy, and various focal therapies. Enucleation alone is curative for about 85-90% of cases with unilateral Rb and no extraocular disease. Intravenous chemotherapy is indicated for bilateral Rb, extraocular disease, or high-risk histologic features after enucleation.

Survivors of hereditary Rb have an increased risk of developing a subsequent malignant neoplasm, for example, sarcoma or melanoma. Treatment with External-Beam Radiotherapy (EBRT) further amplifies this risk, which is heavily dependent on the age of EBRT administration. This risk may be acceptably small for patients older than 12 months. The cumulative risk is roughly 1% per year, reaching 50% at 50 years.
