Health Promotion Clinics: Each month, our staff and trained volunteer nurses set up a mobile clinic in 20 different rent-subsidized apartment buildings to conduct wellness assessments. The Health Promotion Clinics not only help seniors to stay well, but also assist in preventing diseases from escalating to the point where clients will no longer be able to live independently. A client assessment includes: weight, blood pressure screening, pulse, oxygen saturation, blood glucose, medication review, geriatric depression screening, review of health status changes, assessment for substance abuse, nutritional risk assessment, functional assessment (ADLs), assessing health knowledge and willingness to engage in health education and referrals to other area health care resources. Through various screening tools, HealthSET nurses also identify those clients with chronic conditions. For clients with chronic conditions, HealthSET staff and volunteers work with them to create a Personal Health Record that includes: recognizing triggers, communication tools during a doctor’s visit, red flags and medication reconciliation. We ensure each client has a primary care physician (P*P). We often act as a facilitator between the client and the P*P.
Client Advocacy:our Client Advocates work with clients who are faced with serious life challenges such as illness, financial issues and/or the inability to access services. As the healthcare system has grown increasingly complicated, these life challenges often thwart successful health outcomes as patients are asked to follow complicated care regiments, regularly return to a variety of care providers, and successfully cope with a host of challenges along the way. Client Advocates work with clients to achieve these health outcomes. By directly collaborating with health care providers and diverse community resources, the Client Advocacy program prevents premature transitions into long term care facilities as well as helps to reduce clients’ fear and confusion associated with end of life transitions. We ensure they receive necessary healthcare and social services so that they may remain in their homes, cope with complex medical needs, psychosocial crises, and life transitions. The Client Advocates conduct a thorough and standardized needs assessment to prioritize gaps in care and links them with appropriate resources to meet these needs. Following that the Client Advocates provide follow up to ensure that referrals have met the clients’ needs.
Adopt an Elder:This program started because many of our clients receive no cards, gifts, or visits during the holiday season. Therefore, in an effort to help our clients feel remembered and less isolated during the holidays we organize volunteers and coordinate visits so our clients can receive gifts of joy and companionship.
