Sometimes a wide ridge can be deceiving. This anterior mandible, at site #23, had soft, D4 bone. This is often referred to as a 'hollow mandible', and the super soft medullary bone can't be relied on for achieving high torque even with a wider implant with compressive threads. The first implant I placed had low torque (15Ncm).
As my mentor used to tell me, "Sometimes the only way out is to go deeper.."
I placed a 4.3x18mm implant to engage the strong cortical bone at the inferior border of the mandible. This produced 60 Ncm of torque and allowed for immediate loading of the prosthesis.
