Adjustable sedation

Today was another of those days when I’m so grateful to be using Remimazolam in my sedation work. After 500 odd cases ranging from simple 1 hour to 5 hour complex cases: I’ve found the “adjustable” maintenance of sedation to be an absolute game changer.

Classic example today, lots of water spray during treatment, too sedated and the patient keeps coughing. This is very common in dental sedation due to reduction in pharyngeal muscle tone. We don’t want the patient to lose the cough reflex, but we want them to be able to accept treatment and be comfortable.

Reducing the maintenance dose of Remimazolam helps us find the sweet spot where we have adequate sedation and good airway patency.

I often find during longer complex cases that sometimes we need to be lighter, sometimes a bit more sedated. It’s hard to do with Midazolam, very possible with Remimazolam due to short half life and no accumulation.
