Adult Cosmetic Orthodontics – 3D Dentists 12/12/2023

2024 Courses and Dates!

I have two dates set with my 3D Dentists course. This course is geared toward efficiency and scaling your aligner practice. This also includes bringing an assistant, as well as access to my Ortho Foundations course online. Dates for these are Feb 8-9 and Aug 15-16 at the 3D Dentists retreat center in Raleigh NC. The other course I have is through the American Orthodontic Society. This course is more didactic going into the weeds a bit as far as diagnosis and biomechanics of aligner therapy (and orthodontics in general). We also discuss a bit about in-house aligners as well, and include an optional hands on demonstration using Blue Sky Plan. This will be at the brand new AOS training center in Dallas. Dates are March 1-2 and Sept 13-14.

In a nutshell, for folks already doing a bit of aligners but want to become more efficient, lower refinements, set the business systems in place, and scale their aligner practice, the 3D Dentists course is the choice for you.

For those who haven't done much ortho yet or are just getting started, are frustrated after taking an aligner certification course but are still left wondering how to diagnose and treat, or have had very minimal orthodontic training, the AOS course is the better option.

One of my favorite things is interceptive ortho, especially expansion and myofunctional therapy. Pt reports no longer mouth breathing, as well as a heightened sense of smell.
Nasal breathing ✅
Proper arch width ✅

Honored to help this young lady replace a broken front tooth. We were able to take the tooth out, out in an implant right away via a fully guided surgery, and used 3D printing technology to get a tooth on there right away.

The video (will post in the comments) was day of, as well as the first Xray and CBCT. Last Xray with the tibase was her one month check up. Already seeing great bone growth around the neck of the implant.

The GM Helix implant by .us was clutch. The crown material was OnX Tough by . Looking forward to more shades available with their new Tough 2!

Patient shielding during dentomaxillofacial radiography 15/09/2023

Just in case you missed this study in JADA this month: "Patient thyroid shielding during diagnostic intraoral, panoramic, cephalometric, and CBCT imaging should be discontinued as routine practice. As necessary, federal, state, and local regulations and guidance should be revised to remove any actual or implied requirement for routine thyroid shielding for intraoral, panoramic, cephalometric, and CBCT images."

Glad to see the science is backing up what many of us have been challenging for years. Time to reach out to our state and regulatory boards.

Here is a fun case we just finished today. Patient had severely impacted canines. We started with straight wire therapy (including primary teeth) to develop the arches. Once into a rigid arch wire on the lower, we sent to OS for lower canine exposure. Phase 1 took about 18 months to get the canines on track. Then we held in a lower utility archwire while remaining primary teeth were shed and permanents erupted. Then we re-bracketed and went through AWS for about about another 16 months.

These impacted canine cases can take a while, but the results are worth it. And I'm very happy that we were able to treat non-extraction.

Ramping up day 1 and of the 3D Dentists Summit! The energy is electric!

Here is a new prototype fresh out of the cure oven. Designed in Exocad (outsouced the design this time, but made some final tweaks). Printed on the Pro55 in OnX Tough. Optiglaze pink/red on gums, A/clear on teeth. Cured in ProCure2 under Crown profile for 2 minutes (that gets the optiglaze set nicely). Then layered triad over the gums. Will switch to pink composite when I'm all out of triad, unless someone has found a good alternative!Once gums were contoured and shaped, did clearcoat with uncured OnX Tough all over to glaze and seal. Cured in ProCure2 under ONX Tough Profile. Halfway through the cure cycle I removed in, placed it in glycerine, and then resumed the cycle.

It's been asked on whether or not the collapse of the epiglottis would hinder the effectiveness of Mandibular Advancement Devices (MAD) for management of Obstructive Sleep Apnea (OSA). This study shows that it doesn't, and that oral appliance therapy is a viable option for those with epiglottic collapse.

Just finished up this case today. Deep bite, pt is missing lower 6's. Just over 2 years of ortho. Shovel shaped incisors improved with come no-prep injection molded composite veneers. Pretty happy with results. Started when he's 16 yo. Now he can head off to college with a fresh, confident smile. Told him no opening beer bottles with those front teeth though.

Trying something a bit different. Here is a case where I wanted a bit more cantilever than I would do with monolithic zirc. So we designed a bar, which I had milled by Alien Milling Technologies. Then I printed the overlay in SprintRay OnX for the try in. I’ll let her wear it for a few weeks, and if she’s happy I’ll mill the zirc overlay. Then when she comes back I’ll strip the bar and bond the zirc on. I’ll also anodize the bar if needed.

I’ll post progress as we go!

Using iPhone and iPad photography with IOC Snapshot and Open Dental 03/07/2022

This video demonstrates how you could use an iPhone or iPad for patient photos, and how to integrate them directly into Open Dental via the IOC Snapshot app and program. I used to be snobbish about my cameras. Now I just care about what’s most simple to use so my team can be getting them every patient. I would recommend getting a ring light lor a lighting mount geared toward dental photography.

Setup, then Imaging, then Mounts
I set mine 3x3. Also I set the width to 4290, and height to 2800 ala the IOC Snapshot installation manual. This allows for higher res photos, even if in a composition.

I had originally shot this as an internal training video, but figured everyone could benefit from it.

Working on a new workflow for economical lower fixed hybrids when opposing a full upper denture. Working in a rural area, this has been a very appealing treatment option for many. With having an in-house lab, it actually costs me less in both time and parts to do fixed than it does to do a locator overdenture. But I don’t want to do zirc opposing a denture, which leads me to this.

In this case I’m wrapping the Trilor bar in OnX printed on my SprintRay Pro 55. I’ve done this with PMMA before with good results, but this is the first time doing a printed material as a “final”. The patient was on board with giving it a try. To be continued…

I absolutely love being able to print a precise replica of a person’s jaw for my full arch implant surgeries. This here is a tricky case. Vertical Maxillary Excess makes hiding the transition line under the lip impossible. Will have to go with an FP1 prosthesis. Pneumatized sinuses along with mucus retention paeudocysts make sinus augmentation cost prohibitive for patient, as well as zygomatic implants. So we agreed to a shortened dental arch, fixed hybrid.

Printing (on SprintRay) the jaw, which has been segmented by Diagnocat, will allow me try the surgical guide I’ve made in Blue Sky Plan and to a test surgery to make sure I’m dialed in and where I need to be both surgically and for the final prosthetics.

This stuff is fun!

This was a pleasant surprise to find in my email this morning! I've been wanting to use DESS ti-blanks but they aren't supported by my mill. But the good folks at Alien are now offering them for $69.00 including final screw! That's only about $10 more than when I was milling them myself using NT Trading. And now that I'm having supply issues with NT Trading, I was thinking of swithing to Medentka. But now, since I already have DESS scan bodies I'll be giving these a try.

There are a lot of offices dealing with turnover and scheduling difficulties, as well rising cost of doing business. My office hasn’t been immune to this either. With that added stress, it’s easy to put our own mental, physical, and spiritual well being on the back burner. It’s hip to talk about the importance of self care. But I think that term can be a bit misleading. I had come across this quote that I think describes what true “self care” really is.

“Self-care is often a very unbeautiful thing.

It is making a spreadsheet of your debt and enforcing a morning routine and cooking yourself healthy meals and no longer just running from your problems and calling the distraction a solution.

It is often doing the ugliest thing that you have to do, like sweat through another workout or tell a toxic friend you don’t want to see them anymore or get a second job so you can have a savings account or figure out a way to accept yourself so that you’re not constantly exhausted from trying to be everything, all the time and then needing to take deliberate, mandated breaks from living to do basic things like drop some oil into a bath and read Marie Claire and turn your phone off for the day.

A world in which self-care has to be such a trendy topic is a world that is sick. Self-care should not be something we resort to because we are so absolutely exhausted that we need some reprieve from our own relentless internal pressure.

True self-care is not salt baths and chocolate cake, it is making the choice to build a life you don’t need to regularly escape from.

And that often takes doing the thing you least want to do.

It often means looking your failures and disappointments square in the eye and re-strategizing. It is not satiating your immediate desires. It is letting go. It is choosing new. It is disappointing some people. It is making sacrifices for others. It is living a way that other people won’t, so maybe you can live in a way that other people can’t.

It is letting yourself be normal. Regular. Unexceptional. It is sometimes having a dirty kitchen and deciding your ultimate goal in life isn’t going to be having abs and keeping up with your fake friends. It is deciding how much of your anxiety comes from not actualizing your latent potential, and how much comes from the way you were being trained to think before you even knew what was happening.

If you find yourself having to regularly indulge in consumer self-care, it’s because you are disconnected from actual self-care, which has very little to do with “treating yourself” and a whole lot do with parenting yourself and making choices for your long-term wellness.

It is no longer using your hectic and unreasonable life as justification for self-sabotage in the form of liquor and procrastination. It is learning how to stop trying to “fix yourself” and start trying to take care of yourself… and maybe finding that taking care lovingly attends to a lot of the problems you were trying to fix in the first place.

It means being the hero of your life, not the victim. It means rewiring what you have until your everyday life isn’t something you need therapy to recover from. It is no longer choosing a life that looks good over a life that feels good. It is giving the hell up on some goals so you can care about others. It is being honest even if that means you aren’t universally liked. It is meeting your own needs so you aren’t anxious and dependent on other people.

It is becoming the person you know you want and are meant to be. Someone who knows that salt baths and chocolate cake are ways to enjoy life – not escape from it.”
