We are a coalition of healthcare professionals, private citizens and businesses working together to provide information and education on hospice, palliative care, home health care, home care services, and other members of Healthcare throughout the continuum of care. In 2018, the Idaho Quality of Life Association merged with the Idaho Association for Homecare, and became the Idaho Health Continuum
of Care Alliance. Our mission is to work collaboratively with our partners in providing education, tools and resources to promote quality of life and education among the Healthcare community. About IHCCA
Our mission is to bring together healthcare leaders and the community to exchange ideas, provide education and to take action in assisting individuals to receive the highest quality of health and personal care in their Idaho homes and other community settings. We are committed to:
- Allowing individuals to live and age in their residence of choice
- Provide safer transitions of care and education across the continuum of care, while reducing available hospitalizations
- Represent the interest of the acutely ill, chronically ill, disabled and dying Idahoans. A project by the Idaho Healthcare Continuum of Care. Our goal is to engage the entire state of Idaho in conversation and topics that affect Home Care, Home Health, Hospice, and all things post-acute. As we navigate through the spectrum of care for those in the community, this is where you will come for support and inspiration.
