দূর্ঘটনাবশত শরীরের কোন অংশ (হাত/পা/আংগুল) আলাদা হয়ে যাওয়া রোগীর জন্য কতটা দুঃখজনক সেটা যার হারায় সে-ই বোঝে।
এরকম দূর্ঘটনার ক্ষেত্রে সময়মত ব্যবস্থা নিলে অনেক ক্ষেত্রে আলাদা হয়ে যাওয়া অংশ পুনরায় জোড়া লাগানো সম্ভব।
আমরা এরকম পরিস্থিতির সম্মুখীন হলে কি করা উচিত সে সম্পর্কে কিছু কথা বলছি যা আমাদের সবারই জানা প্রয়োজন। আমি মূলত দুটো বিষয় হাইলাইট করব -

১) কিভাবে কেটে যাওয়া অংশ সংরক্ষণ করে রোগীর সাথে স্পেশালাইজড সেন্টারে পাঠানোর ব্যবস্থা করব?
২) আমরা রি-ইম্পল্যান্ট সার্জারির জন্য বাংলাদেশে(ঢাকা) কোথায় রেফার করতে পারি?

First aid to a patient with amputation:

1. Check the person's airway (open if necessary); check breathing and circulation. If necessary, begin rescue breathing, CPR, or bleeding control.
2. Try to calm and reassure the person as much as possible. Amputation is painful and very frightening.
2. Control bleeding by applying direct pressure to the wound. Raise the injured area. If bleeding continues, recheck the source of the bleeding and reapply direct pressure, with help from someone who is not tired. If the person has life-threatening bleeding, a tight bandage or tourniquet will be easier to use than direct pressure on the wound. However, using a tight bandage for a long time may do more harm than good.

**How to preserve the amputated part & transport the part with patient--
--There are two methods of preservation. I am talking about the most accepted method of preservation preferred by Plastic surgeons/Hand surgeons worlwide which has no controversies.

1. Gently wash the severed limb with sterile water or saline solution but do not scrub.
Do not put an amputated limb directly in water/saline, as this may cause damage that could hinder/interrupt reattachment.

2. Wrap(not too tightly) the limb in a moist, clean material. Sterile gauze soaked with sterile saline solution or sterile water is best. If you do not have gauze, compromise by wrapping the limb in the cleanest absorbent material available. Paper towel will work if you do not have gauze or a clean cloth.

3. The put the amputated part in a dry & watertight plastic bag/container.
4. After that put the plastic bag/container in a plastic bag/box which contains ice.

(DO NOT put the severed part directly on ice. DO NOT use dry ice as this will cause frostbite and injury to the part)
5. At last transport the limb with patient to a specialized centre.

The amputaed part may survive 4 to 6 hours if it is not cooled. But it may survive from 24 to 30 hours if preserved in above mentioned method.
