- TISSUE TIP -
Harvesting soft tissue grafts from the palate requires a good understanding of anatomy and surgical finesse. Whilst there are many considerations when choosing a donor site, incision design and potential thickness, it is vitally important to respect the greater palatine artery and nerve. They exit through their foramina and travel within a groove in the palate.
One must be extremely aware if you are dealing with a shallow palate as it will be significantly closer. On average, to keep a safety margin I will incise 3mm away from the gingival margin and ensure there are no incisions beyond 10mm from the gingival margin. Naturally, you must be aware if the tooth has recession palatally and don’t forget to probe the teeth and ensure you are not incising near a periodontal pocket.

Highlighted the importance of caries detection dye. This one just kept on giving and giving and needed more refinement beyond this prep picture. The combination of disclosing solution, caries detection dye , air-abrasion, illumination, magnification and photography helps in every stage of bacteria removal. Our eyes do not always spot everything 🙈
Rebuild with TorVM titanium matrix, liquid dam & wedge, venus pearl A2 & OLC, empress direct enamel A2, ochre tint 🙂

There are so many different matrices & rings available on the market that it is important to understand the nuances of each. When faced with tricky scenarios such as deep floors and missing cusps, I’ve grown a liking to this beautifully curved titanium matrix in combination with liquid dam (both supplied by ). Normally I’ll use a plastic wedge, but this one was fully sealed and stable without 😎
