We are trying to save those implants that were placed by a colleague some years ago.

Soft tissue were not great.

Saving tooth and implants we think is important when possible!

In this case and myself had this referred patient.

The patient had an Implant fracture and screw fracture in two adyacent implants .

Extracting these two broken screws can be a nightmare.

Once the implant are removed the problem is the bone defect that is left.

Usually is needed to perform a bone graft procedure which requires some months to wait for the implant placement. And after the implant placement we would need to wait another 3 months of osteointegration period.

In this case we did it in only one time surgery and one surgical site. We didn’t open the mandible to harvest a bone block.

We remove the broken implants, perform the bone grafting procedure, place the implants and just wait for the second stage surgery.

So in 5 months the case was ready for the prosthodontics.

🇺🇸 These kind of big bone defects have some steps that are mandatory:

1.-remove the previous implants
2.- Wait, there is no hurry! We need the tissues to be healed
3.- Bone reconstruction
4.- Implant placement
5.- Soft tissue reconstruction
6.- Second stage surgery, usually with more soft tissue

And then send the case to the prosthodontics.

Usually the case is completely finish after 10-12 months (unless you have a pandemic meanwhile )

🇪🇸 Este tipo de grandes defectos óseos tienen una serie de pasos que son obligatorios:

1.- Retirar los implantes previos
2.- dejar que curen los tejidos, muy importante
3.- reconstrucción del hueso perdido
4.- Colocación de los nuevos implantes dentales
5.- reconstrucción del tejido blando
6.- cirugía de segunda fase, normalmente con más tejido blando

Y por último enviar el caso al especialista en prostodoncia

Normalmente este tipo de casos tardan en solucionarse más o menos en 10 meses (o más si nos pilla una pandemia mundial en medio)

We are placing implants right after an extraction in 90% of the cases.

It is just needed a little bit of practice. In the aesthetic zone if you want to keep the volume you should do something.

In this case we placed a connective tissue graft.

🇺🇸 One of my favorite sites to harvest it’s the tuberosity, there is good soft tissue, quite a good amount of bone, it is a really fast harvest and almost has no post op.

🇪🇸 Uno de mis sitios favoritos para tomar injertos es la tuberosidad. Hay buena calidad de tejido blando y una considerable cantidad de hueso. Además es muy rápido y prácticamente no tiene post operatorio.

Después de más de un lustro desde el ultimo curso que hicimos con paciente volvemos a sacar otro y yo.

Serán 4 días intensivos operando con nosotros y realizando aquellas técnicas que quieras aprender / mejorar.

De nuevo son solamente 4 plazas (aunque ya quedan 3) pues creemos firmemente en la formación de alta calidad y exclusiva.

Excepto bloques/Khoury, más adelante sacaremos una plaza solo para esto.

Técnicas tan predecibles y asequibles como el injerto libre epitelial es fundamental para mejorar los tejidos para este tipo de casos de zonas mandibulares donde no es difícil que no haya suficiente tejido queratinizado.
