Dientes no restaurables por caries interproximal.
Muchas veces no se usa el hilo dental por su dificultad en los dientes superiores pero el no hacerlo puede sentenciar tus muelas a una caries no restaurable y terminar perdiendo ambos dientes como sucedió en este caso. 😞😞😓😓😭😭

Extracción de muela del juicio con dilaceración en la raíz.
Las muelas del juicio suelen ser erráticas en forma, en este caso presentó una forma pequeña y la dilaceración de la raíz, que es esa curvatura en la punta. Si no se tiene cuidado al sacarla puede fracturarse.
También podemos observar la amalgama filtrada y la caries interproximal (a un costado) por falta de higiene. Lo cual es común en estas muelas ya que lograr usar hilo dental y un correcto cepillo se dificulta por la posición.

El problema de las restaursciones metálicas es que si no se trabajan adecuadamente tanto el técnico dental como el dentista queda estos espacios entre la restauración y el diente. A lo cual nos referimos como "restauración filtrada".
En este caso aparte el punto de contacto entre ambas restauraciones era inadecuado ya que el paciente no podía limpiar adecuadamente la zona, lo que generó caries. El premolar fue el que retiramos ya que era el diente que presentaba dolor, aunque ya tenía endodoncia (mal hecha porque se veía la gutapercha sin condensar), el dolor provenía de la encía inflamada.
Por eso es importante que los puntos de contacto entre dientes estén libres para una adecuada limpieza con el hilo dental y evitar estas situaciones.
En cuanto al molar el paciente no quiso cambiar la restauración aunque estuviera fracturada y filtrada 😓, muchas veces pasa esta situación.

Por ello hubo que sacar el diente.

La pérdida de hueso lleva a la perdida de los dientes 😞 por ello es importante hacer visitas periódicas al dentista para evaluaciones periódicas.

En este caso las caries son interproximales (entre dientes) cuya causa principal es la falta de higiene al no usar el hilo dental.
