Also known as; Female pattern hair loss (FPHL)

FPHL is a distinctive form of diffuse hair loss that occurs in women with androgenetic alopecia.

FPHL has a strong genetic predisposition.
It is more common after menopause.

A few women with FPHL are found to have excessive levels of androgens. These women tend to suffer adult acne, irregular me**es and excessive facial hair.

Managing FPHL

☑️ A combination of topical and hormonal treatment may be prescribed-long term treatment is usually necessary

Is your skin aging faster than his?

The difference between male and female skin is the reason aging is different between the 2 genders:

Both men and women lose about 1% of their collagen per year after the age of 30.
For women this escalates significantly in the first 5 years after menopause, may result in a loss of 2% of the collagen per year.

Collagen and elastin don’t degrade as rapidly in male skin because of androgens. In addition testosterone improves elasticity & skin hydration.

Alopecia Barbae/ Beard Alopecia

A specific form of Alopecia Areata( see previous post)
It occurs when immune system mistakenly attacks hair follicles.

Pseudo folliculitis barbae aka razor bumps

Is a common inflammatory reaction of the hair follicle, most often on the face as a result of shaving.
May also occurs on armpit, p***c area and legs.

Who is at risk?

How to recognize this condition?
🪒 ingrown hairs which may be itchy or painful on the face(jawline) and neck

Acne scarring

It affects 30% of those with moderate or severe acne vulgaris. More commoner with those with nodulocystic acne.

Different types of acne scars:
▫️Ice-pick scars- deep, narrow and pitted
▫️ Rolling scars- broad depression with slopping edge
▫️ Boxcar scars- broad depression with sharp edge
▫️Atrophic scars- flat and thin
▫️ Hypertrophic or keloid scars

Winter itch

▫️A type of sub clinical dermatitis, affect predisposed individuals during cold weather.

▫️Affect healthy individuals of all ages but more common in older people with dry skin.

▫️The cause is unknown but has been associated with:

🔹dry skin

🔹fabrics such as wool

▫️Commonly affect the legs, spares face, scalp and hands.

▫️The itchiness is sudden in onset, noticeable at night or when removing clothing.

▫️Managing winter itch;

☑️ Bath in warm water

☑️ Apply a moisturizer

☑️ Wear light clothing and non irritating materials like silk, cotton and linen.

☑️Avoid irritating material like wool

Paronychia is inflammation of the skin around a finger or toe nail. Acute if it’s been present for less than 7 weeks and chronic if persists for more than 6 weeks

Who gets affected:
✋🏾 Nail biters
✋🏾 People who suck their fingers
✋🏾 May occur after a manicure/ pedicure especially if instruments used are not adequately sterilised
✋🏾Ingrown nails
✋🏾 Use of certain medication such as retinoids
✋🏾People who constantly have cold and wet hands such as fishermen, dairy farmers, bartender, cleaners

The cause:
✋🏾Viral, bacterial or fungal infections
✋🏾 Chronic dermatitis of the nail fold which may be complicated by different infections

✅Treatment is directed to the most likely cause

Complications such as nail abnormalities (nail dystrophy) and cellulitis sometime occurs.

Fungal infection of the nail
Dermatophytes, mould and Candida may affect the nail which then leads to this condition.

It may affect one or more toe/fingernails. The big toenail and the little toenails are often the commonly affected.

Occasionally nail clippings are done to confirm the diagnosis and identify the organisms causing the infection.

Treatment of choice are systemic antifungals, which may go on for several weeks.
*Fingernail infections are usually cured more quickly than toenails.

Melasma
Melasma is a common acquired skin disorder that presents as a bilateral, blotchy, brownish facial Pigmentation.

Who gets Affected?
🔘 more common in women than in men
🔘 onset typically between the ages of 20 and 40 years.
🔘 most common in people who tan easily or
have naturally brown skin.
🔘 It is less common in people with fair skin or darker skin.

The cause of melasma is complex:
🔘photoaging disorder in genetically predisposed individuals.
🔘 Family history
60% report affected family members
🔘Sun exposure
ultraviolet and visible light promote
melanin production
🔘Hormones
pregnancy and the use of
estrogen/progesterone-containing oral
contraceptives, intrauterine devices, implants, and hormone replacement therapy, are implicated
🔘thyroid disorders can be
associated with melasma

❕Melasma presents as bilateral, asymptomatic, light-to-dark brown macules or patches with irregular borders.

Should be differentiated from:
🔘post inflammatory hyperpigmentation
🔘drug induced hyperpigmentation
🔘 Nevus of ota, solar lentigo/ freckles

Managing Melasma
A combination of measures is generally required.
❕sunscreen all year round
❕Discontinue hormonal therapy where possible
❕Topical treatments such as tretinoin, hydroquinone, ascorbic acid…
❕Oral treatment
❕procedural techniques-laser, microneedling

Tinea Capitus AKA scalp ringworm

Tinea capitus is a fungal infection of the scalp, involving both the skin and hair.

🔅predominately affect preadolescent children
🔅may affect adults, especially those who are immunocompromised

Risk factors:
🔅household crowding
🔅animal contact
🔅warm humid environments
🔅contact sports

Clinical presentation
🔅non inflammatory variants
•fine scaling

•pustules, bogginess, kerion

May be mistaken for:
🔅seborrhoeic dermatitis
🔅impetigo
🔅scalp folliculitis
🔅Lupus

Treatment
🔅antifungals

Exogenous ochronosis (EO) is a cutaneous disorder characterized by blue-black pigmentation resulting as a complication of long-term application of skin-lightening creams containing hydroquinone but may also occur due to topical contact with phenol or resorcinol in dark-skinned individuals and with certain medications such as quinine.

May mimic melasma;
•presents with hyperpigmentation on sun exposed areas especially around the cheek areas.
•The lesions may range from gray, blue-black macules to papular nodular lesions depending on the stage of the disease. Swipe⏩

‼️Treatment is predominantly preventive
‼️ Exceedingly difficult to treat!!

Managing Ochronosis
▫️early diagnosis
Not to be mistaken for melasma where hydroquinone might be used- this will worsen the condition
▫️stop offending agent.
▫️Avid sun protection
▫️Retinoids, glycolic acids and antioxidants are among pharmacological treatments that can be used
▫️Procedural treatments like chemical peels, certain lasers might be helpful

‼️Only use implicated products only if prescribed to you by an expert in skin care.
