Today’s Neuro focused article we take a look at Cognitive Decline in Parkinson's Disease patients.

Parkinson's disease (PD) is widely recognized for its impact on motor functions, but it also affects cognitive abilities over time. Cognitive decline in PD is characterized by difficulties with memory, attention, executive functions (such as planning and decision-making), and visuospatial abilities. This cognitive deterioration can lead to Parkinson's disease dementia (PDD), which typically develops in the later stages of the disease.

As Parkinson's disease progresses, cognitive decline can significantly affect a patient's daily life. Individuals may struggle with tasks that require concentration, problem-solving, or multi-tasking. Memory issues may lead to forgetfulness, and difficulties with executive functions can impair decision-making and the ability to carry out complex activities. These cognitive challenges can lead to increased dependence on caregivers, reduced ability to live independently, and diminished quality of life.

While physiotherapy is traditionally associated with managing motor symptoms, it can also play a role in addressing cognitive decline in Parkinson's disease, particularly when integrated into a comprehensive care plan that includes other therapies (e.g., occupational therapy, speech therapy, and cognitive behavioural therapy).

Home-visiting physiotherapy benefits cognitive decline by focusing on:
- Cognitive-Motor Integration
- Improving Attention and Focus
- Routine and Familiar Environment
- Holistic Approach
- Caregiver Support

Cognitive decline in Parkinson's disease is progressive and cannot be entirely reversed. However, with targeted interventions, the rate of cognitive decline can be slowed, and patients can maintain better cognitive function for longer periods.

Home-visiting physiotherapy, as part of a multidisciplinary approach, can help manage these symptoms by promoting cognitive-motor integration, encouraging mental stimulation, and providing a structured routine that supports cognitive health.

This months Neuro focus is on the long-term complications of Parkinson’s Disease. Today we look at Motor Function Decline.

Parkinson's disease (PD) is a progressive neurological disorder primarily affecting motor functions due to the degeneration of dopamine-producing neurons in the brain. Over time, individuals with PD experience a decline in motor functions, which manifests as symptoms such as bradykinesia (slowness of movement), muscle rigidity, tremors, postural instability, and gait disturbances. As the disease progresses, these symptoms can severely impact a person's ability to perform daily activities, reducing their quality of life.

In the long term, motor function decline in Parkinson's disease can lead to significant disability. Patients may experience difficulties with walking, maintaining balance, and executing fine motor tasks. These impairments can increase the risk of falls, contribute to a loss of independence, and lead to secondary complications like muscle atrophy and joint contractures due to reduced mobility.

Home-visiting physiotherapy services can play a crucial role in managing motor function decline in Parkinson's disease. Physiotherapy interventions are tailored to the individual's specific needs and disease progression, focusing on:
- Improving Mobility
- Enhancing Balance and Posture
- Gait Training
- Pain Management
- Education and Support

While Parkinson's disease cannot be cured, the decline in motor function can be managed and, in some cases, improved through consistent physiotherapy. Regular sessions can slow the progression of symptoms, enhance functional mobility, and improve overall quality of life.

Plantar fasciitis is a common and painful condition among trail runners, caused by inflammation of the plantar fascia, a thick band of tissue that runs across the bottom of the foot and connects the heel bone to the toes. Repeated stress from running on uneven and challenging terrains can lead to microtears in the plantar fascia, resulting in sharp heel pain, especially with the first steps in the morning or after prolonged activity. Factors such as improper footwear, poor running mechanics, and tight calf muscles can contribute to this condition.

Healing from plantar fasciitis and getting back on the trails requires a structured and personalized rehabilitation program. Home-based physiotherapy aims to provide pain relief, stretching, strengthening exercises, and correcting running mechanics. Our physiotherapists can create a customized treatment plan tailored to your specific needs, guiding you through the recovery process to ensure a swift and complete return to trail running.

Home-visiting physiotherapy services

Welcome to Physiotherapy @ Home. A service provided by Hector,Naidoo & Associates, servicing patients in the suburbs of Cape Town, Johannesburg and Port Elizabeth. We offer this be-spoke healthcare service in the comfort of your home, old age home facility or even at your work.

At Physiotherapy @ Home, we view things a little differently. Our therapy treatment sessions can take place in the home, office, local fitness centre, pool, community centre, park or even the grocery store because life does not happen in a clinic. We believe when patients receive 1 on 1 therapy in their own environment: compliance is higher, outcomes are better, and patients are happier. We believe it is not enough for patients to be independent and functioning at home if they can’t get back in to the community doing the things they love such as: shopping, eating out at a restaurant, going to a sports game, or visiting a museum. We believe patient outcomes are maximized by bridging the gap between home health care and a traditional outpatient clinic by providing outpatient therapy in their home after they discharge from home health services.
