The connection to failure likely comes from a phrase used in Italian theatre: "far fiasco" or "fare fiasco," which translates to "make a bottle." This expression was used to describe a performance that went disastrously wrong.
Some believe that in the Venetian glassmaking industry, a flawed or misshapen bottle would be considered a fiasco, and thus a failure. This idea may have crossed over into theatrical language.
