Is your support coordinator helping you enough to understand your plan and guide you to services that are available for you??
Your support coordinator should do the following for you:
- Explain the NDIS plan and the supports that will help you pursue your goals.
-Identify alternative solutions, and link you to related disability services.
-Liaise with you, your family, friends, LAC, support workers, and other involved services to ensure you receive the best possible outcomes.
-Track your goals and help you throughout your NDIS plan
-There to listen to you and to your challenges to better minimise any obstacles that are eliminating you from achieving your goals.
Make sure your support coordinator is doing the above for you! Its your plan -Its your choice!
