DRY NEEDLING πŸͺ‘

Dry needling is a great technique as part of a robust treatment and rehabilitation plan.
It’s useful for reducing pain and muscle trigger points.

This may assist with your injury recovery or postoperative rehabilitation.

Elbow Fracture

Olecranon fracture stabilized using a tension band wire. Clever technique which creates a compression (closing force) at the fracture site when in flexion. After the surgery and 2 weeks in a long arm back slab set at 90 deg. Management was to cast in elbow flexion set at 90 degrees for 4 weeks. Then onto rehab to re-gain range of motion and strength. Final X-ray is shown here. Well healed and patient has achieved full range, around 3 months post-operation. Great result!

TENDON HEALING

A clear understanding of the healing timeframe a tendon requires is essential when planning rehabilitation.

The 3 images shows the tendon remodeling from weeks 1, 3 and 8 respectively.
Tendon healing is divided into 3 overlapping phases.
1) inflammatory phase - initiated by the injury/trauma to the tendon. Lasting for 3 - 7 days while type 3 collagen increase
2) proliferative phase - increasing amount of intrinsic fibroblast in the tendon building type 3 collagen lasting for 6 weeks
3) remodeling phase - tendon strength increases, the collagen is organised longitudinally and type 3 collagen is replaced by type 1. This will mature over the next year! With the collagen density and tendon strength increasing

A healed tendon regains 63% of its strength after 8 weeks.

Rehab and specific strength exercises facilitate this process πŸ’ͺ🏼

STRENGTH

An integral part of any rehabilitation program is strength training πŸ’ͺ🏼

This is typically commenced once range of motion is restored and symptoms are controlled.

Rehab focused training can initially be designed with a simple structure 2-3 times per week, using light resistance and body weight. Adding more weight as the injury improves.

Over time and in line with specific goals, strength training intensity and frequency will change. This will have blocks of training focusing on different areas - muscle building, strength and power. Linking with an activity, lifestyle or sporting goals. Generally this will be performed in a gym setting with varying resistance, but it will always be tailored to the individual πŸ‘πŸΌ

CASTING

Our patient was placed in a water resistant cast following wrist surgery. To allow for the tendon repair to heal.

After 4 weeks it was time to remove the cast and replace with an Exos wrist brace. Which is a heat moulded and adjustable. A great option to allow us to now start treatment and rehabilitation on the hand, wrist and forearm πŸ‘πŸΌ

PRP

Platelet rich plasma injections are used for treating chronic tendon and joint issues. Such as wrist extensor tendinopathy, knee joint arthritis any many more.

This procedure is performed by a specialist - sports physician or interventional radiologist. Usually 2-3 treatments are required and the key is performing treatment and rehabilitation following the injection.

In the video we see the blood after it has been put into the centrifuge and spun at 2500 revs.

The blood now has 3 distinct layers:
1) Top - Plasma (55%)
2) Mid zone (interface) - White blood cells and Platelets this is what is used for the procedure (4%)
3) Bottom - Red blood cells (41%)

The middle layer is drawn and used for the procedure.
