Does the Invisalign Palatal Expander (2024) show to be an effective skeletal expander in early orthodontic treatments? Shot in in-office on actual live orthodontic patient who had severely rotated (butterfly) incisors with curved contact surfaces. The patient required IPR/Stripping/Reproximation to manage unaesthetic black triangles, create space to derotate and align teeth as well as reduce overjet.

IPR suggested instruments:
Handstrip used - Blue handtrip for IPR by Kerr/Ormco 0.1 mm coarse
Handpiece used - Space File by Dentsply Sirona with oscillating IPR saws

Our objective was to also provide a Class 1 ideal finish. Whitening with take-home gels were prescribed when patient was nearing the end of Invisalign treatment. Total treatment time 18 months. Missing upper lateral incisors are not uncommon in orthodontic patients, especially in females. It can be unilateral or bilateral. In this case, treated by Dr Vandana Katyal (Orthodontist, Sydney AUS) an informed consent was obtained to close the space for the upper missing lateral incisor by natural substitution with an upper canine rather than an implant. Since upper canines have buccally pronounced roots, it is important to add extra palatal root torque in the orthodontic biomechanics plan.

What a fantastic result was achieved here for this lovely patient, treated by Dr Vandana Katyal, Orthodontist. Given she had a bimax protrusive profile and ectopic upper canine, initially extractions were considered. Following discussions, we decided to use interproximal reduction and dental arch expansion instead with clear aligners (Invisalign) and virtual monitoring (DM). We also doubled the number of aligners in each set to improve predictability of complex movements and slow down tooth movement velocity for a more optimal biological soft tissue response.

A fantastic result in about 2 years and just in time for her wedding!
