TFCC Testing ๐ช๐
The wrist weight bearing test is a great way to test for a TFCC tear or injury by measuring how much weight you are able to tolerate through the wrist and hand.
Wrist Weight-Bearing Test: The individual is in a quadruped position while they place their palm on an analogue scale. The elbow is in full extension over the wrist with the long finger at 12 oโclock to prevent radial or ulnar rotation. They then push down on the scale until pain is felt at the ulnar side of wrist but not exceeding pain or until maximum amount of pressure is reached. The average of three trials are taken and testing is done to the affected and non-affected sides with and without non elastic tape/WristWidget.
Injured wrist test results with non elastic tape/WristWidget:
0-45 lbs or 0-20 kg = severe TFCC tear
45-65 lbs or 20-29 kg = moderate TFCC tear
65-100 lbs or 29-45 kg = mild TFCC tear

โฆPatients who have a TFCC tear often develop symptoms in their ECU. When the TFCC is treated, the ECU symptoms resolve. There are also many cases that have been diagnosed as a TFCC injury which are only an ECU issue.
The ECU subsheath is critical for extensor carpi ulnaris tendon stability and part of the triangular fibrocartilage complex which is why an MRI may show tendonitis, a tear, or TFCC tear and is often misdiagnosed.
When the TFCC is involved, the ECU takes a sharper turn because the DRUJ spreads in TFCC injuries. This causes a common secondary complication to the ECU. Normal range for ulnar deviation is 30-40 degrees.
How do I know the difference? Start with the weight bearing test! If your weight bearing tolerance is normal, then the TFCC is fine and you want to look at the ECU.
ECU ONLY
-No pain on weight-bearing
-Healthy and injured wrist test the same on the Weight Bearing Test
-Wrist aches after wearing the WristWidgetยฎ
-Clicking or popping and pain at ECU insertion
-The WristWidgetยฎ will NOT be helpful in recovery
TFCC ONLY
-Pain on Weight Bearing
-Injured wrist tests lower than healthy wrist on the Weight Bearing Test
-Injured wrist tests higher with WristWidgetยฎ/taping than without
-The WristWidgetยฎ will be helpful in recovery
TFCC-ECU COMBO INJURY:
-You know you have a TFCC tear when there is a loss of weight-bearing tolerance and an immediate increase with tape or the WristWidgetยฎ.
-You know that the ECU is involved if you have pain to touch at the insertion of the ECU at the back of the hand when the wrist deviates towards the ulna (ulnar deviation).

TFCC tears donโt cause loss of motion or swell. However, a fracture or other injury can accompany a TFCC tear if there is swelling.
If your wrist is fractured, it must be cast so the bones can heal properly first. The following treatment plan is for wrists WITHOUT a cast, or after the cast has been removed.

20kg is considered a โfunctionalโ wrist - yay! This means you should be able to function around the home, for the most part. Things like dishes, dressing, making a bed etc. should not hurt.
This does NOT include carrying children, animals, heavy bags of groceries etc. This is a light functional wrist.

โMild Injuryโ means your wrist is stable and you can load it for athletic endeavors!
Donโt let this give you a false sense of security - you're not quite healed. Beginning to exercise too soon will set back your healing process!
Your goal is to take 2-4 weeks to focus on the imbalances that this injury caused and there are many, especially when you have had the injury for a long time.
In this healing phase, you should wear the WristWidgetยฎ until your weight bearing is normal (equal to your uninjured wrist) and there is NO clicking. You are not done with the healing process yet.
This phase is important because often people think that they are healed and go back to normal functioning without the WristWidgetยฎ. They fall back below 65lb/29kg for weight bearing tolerance and symptoms resurface.
