* valid photo identification that matches the name listed on your document(s). Drivers licence and/or passport are accepted. We cannot accept Ontario Health Cards as a form of identification.

* Your document(s). Please make sure that the document(s) are completed but not signed. You must sign with me at the appointment.

For the first document - $40.00 plus HST.
