Case Presentation: 37yo pregnant female with lockjaw

A couple of weeks ago I had a 37yo female in her second trimester present with jaw pain and restricted opening to 1 knuckle and I thought I would share this clinical win!

🌱After a thorough history and physical assessment a trial of conservative care 2x/week over the next 2weeks was recommended that included soft tissues therapy, joint mobilizations, education/reassurance regarding her specific condition and exercises.

🌱Following treatments the patient would have moderate relief of pain and transient improvement in jaw opening ranging from 1.5-2 knuckle spaces for approximately 1 day. After 2weeks of care under this treatment plan and trial of acupuncture was recommended.

🌱Following the patients first acupuncture session she was able to open her jaw 2.5 knuckle spaces and had a significant decrease in pain intensity. Following her second session of acupuncture she was able to open her jaw completely without pain! 🙌🏻

Chiropractic care + pregnancy 🌿

At least 50% of pregnant women will experience back pain. Pregnancy-related back pain can be multi-factorial + rarely only has one cause. In addition to supporting moms with the aches + pains that may arise during pregnancy, chiropractic care can also help to prepare for childbirth.

As baby grows and the uterus expands, the abdominal muscles stretch + loose their ability to support as they did. Chiropractors are able to assess moms biomechanical alignment in addition to muscle + ligament tension to help aid in pain relief + optimal baby positioning.

During the third trimester, levels of the hormone “relaxin” increase + contribute to the loosening of joints in order for the pelvis to accommodate for enlarged uterus, the baby and later birth. However, the loosening of joints can decrease joint stability + further alter posture. Other structures including musculature may be stressed, resulting in a predisposition to pain + injury.

Chiropractors can help to:

🌿Reduce pain and improve function through spinal manipulation, gentle mobilization of joints, soft tissue therapy + targeted exercises

🌿Relieve fatigued, aching muscles through progressive, targeted rehab to support you + your body

Today, the Ontario government issued a province-wide, stay-at-home order, effective Thurs., Jan. 14, 2021, at 12:01 a.m. The order requires everyone to remain at home, except for essential purposes, such as accessing health care services or grocery shopping.

There are no changes to rules around regulated health care professionals, such as chiropractors, registered massage therapists and physiotherapists.

According to Ontario’s Reopening Ontario (A Flexible Response to COVID-19) Act, 2020 (effective Jan. 11, 2021), businesses that can continue to remain open include: health care services provided by regulated health professionals (point #53); organizations that provide health care, including clinics and independent health facilities (point #55) and manufacturers, wholesalers and businesses that provide logistical support of or for products and/or services that support the delivery of health care in all locations (point #58).

Patients and members of the public will be able to continue to visit their chiropractor during their regular hours of operation. And chiropractors will continue to comply with strict PPE and safety precautions to protect their patients and team members.

Born in Oshawa, Dr. Taylor Warman attended the University of Ontario Institute of Technology where she obtained her Bachelor of Biological Science, specializing in Environmental Toxicology. Upon completing her undergraduate degree, she moved to Toronto to attend the Canadian Memorial Chiropractic College (CMCC) and graduated in 2020.

In practice, Dr. Warman’s goal is to determine the root cause of your issue, and to create individualized treatment plans that are aligned with your health goals. She is diversely trained to use a combination of chiropractic manipulations, gentle joint mobilizations, myofacial release therapy, rehabilitative exercises, and contemporary medical acupuncture to get you back to doing what you love.

Dr. Warman’s a firm believer that movement is medicine! In her free time, she enjoys discovering new local gyms and trails to hike with her friends and family! Her favourite ways to sweat include spinning, HIIT training, and yoga.

Happy Halloween everyone! 🎃

Here is some spooky skeletal system trivia I thought you might enjoy 👻. Check out my story before reading if you want to test your knowledge!

1️⃣Our skeleton is made of approximately 206 bones: This number can vary from person to person believe it or not! For example some people can be born with an extra rib or lumbar vertebrae. Cool!🤯

2️⃣Babies are born with approximately 300 bones: Babies are made with many cartilaginous bones that grow and fuse together into our mature adult bones. 👶🏽

3️⃣We have one bone not connected to any others: The hyoid bone, which is in your anterior neck, is responsible for anchoring the tongue, helping us chew, swallow, and plays a role in voice articulation as well.😛

4️⃣The smallest bone in your body is found in the ear: this bone is called the stapes and is only 3mm long. It helps to translate sounds along with the malleus and incus into vibrations that the brain can understand. 👂

5️⃣There are 2 types of bones: cortical bone helps to form the primary outer “structural” part of our bones, and trabecular or “spongy” bone which is found on the inside of large bones. 🦴

6️⃣The funny bone is not actually a bone: The “zing” we get when we hit our “funny bone” is actually caused from irritating the ulnar nerve that runs in a groove on the inside of our elbow and innervates the muscles of our forearm and hand. 💥

What’s up with instrument assisted soft tissue mobilization ⁉️

Instrument Assisted Soft Tissue Mobilization (IASTM) is a supplemental treatment used along side manual therapy, exercise and other modalities to help get patients moving and feeling better! 🤸🏾This treatment modality is the modern evolution of the Gua Sha technique used in Traditional Chinese Medicine.

✨Fun fact✨Gua Sha was not used to treat musculoskeletal conditions as IASTT is today, but rather for the purpose of moving bad chi through the skin along meridians. You may have more recently see beauty gurus on YouTube using Gua Sha to boost blood circulation and promote lymphatic drainage before makeup application 💁🏻‍♀️😅

For clinical purposes, IASTM works through the introduction of controlled microtrauma to the tissue so that a local inflammatory response occurs 🩸.This local inflammatory response initiates reabsorption of inappropriate fibrosis or excessive scar tissue and facilitates a cascade of healing activities resulting in remodelling of affected soft tissue structures.

BENEFITS:
✨increased fibroblast proliferation for collagen production
✨reduction in scar tissue and adhesions
✨promotes healing through increase blood flow
✨remodelling of unorganized collagen fiber matrix

I took the certification while in school so that I could learn the best methods of using these tools with my patients. While there is still more research emerging on the use of IASTM and its effects, through clinical experience I find this to be a great tool that not only helps to save my hands, but that patients enjoy and find benefit from!

✨HEADACHES ✨

Headaches are something I am particularly passionate about since experiencing regular headaches and migraines for years 😓

Head pain is no joke, my migraines have stopped me from going to work/school, hanging out with friends, studying for big exams, going to the gym, and just doing my regular activities of daily living. By working with my chiropractor and medical doctor, I have been able to decrease the frequency and intensity of my headaches, but sometimes they catch me off guard! In cases like this, these are my TOP natural at-home remedies that have helped to alleviate some of my discomfort so I can go about my day:

1️⃣PEPPERMINT ESSENTIAL OIL: I use the roller, but any peppermint essential oil will work.

2️⃣COLD/HEAT COMPRESS: place an ice pack on your head and heat on your feet (I use a microwaveable rice bag).

3️⃣REST/MEDITATE: take sometime to close my eyes, breath and de-stress.

4️⃣MOVEMENT: as much as I don’t want to at the time, a short walk or movement always helps me.

5️⃣GINGER: I take a ginger capsule to help alleviate some of the nauseas associated with my migraines.

6️⃣MAGNESIUM: rubbing magnesium cream on my shoulders and neck helps to decrease muscle tension.

7️⃣HYDRATE, HYDRATE, HYDRATE

Is back pain common? YES! Does that mean its normal? Heck no!

Experts estimate that as much as 80% of the population will experience a back problem at some time in their lives. In fact, low back pain is the current leading cause of disability worldwide! 🤯

World Spine Day highlights the importance of spinal health and well being. It’s a reminder to get back to the basics when looking for ways to maintain spinal health and prevent future issues. It promotes PHYSICAL ACTIVITY 🏃🏾, GOOD POSTURE🧘🏻, RESPONSIBLE LIFTING🏋🏽‍♀️, and HEALTHY WORKING CONDITIONS💻 🔨 as ways to achieve this.

Back pain can come from a number of difference sources, so its important to check in with your health care provider if the pain is particularly aggravating or persistent.

MOVEMENT IS MEDICINE! You would be surprised what a walk around the block will do for your achy body. This is your reminder to show your spinal some love and incorporate movement into your daily routine!

Feeling extra grateful since completing my final board exam last weekend!🙌🏻 It’s been a long 8 years of post secondary education, but I honestly can’t believe I’m already here at the same time! I’ve spent the week organizing and preparing to begin accepting new patients at a clinic near my hometown under my provisional license (location TBA soon🤫), and I couldn’t be more excited! 😁

Things that I am feeling particularly grateful for:
🍂friends and family that support me and lift me up
🍂 the ability to pursue a career that challenges me and fulfills my passion for helping others
🍂 my health, and the health of my loved ones
🍂 that I live in a country with 4 seasons (fall is BY FAR my favourite season)

What’s that popping sound⁉️

So glad you asked!

An adjustment is a precise technique use by chiropractors to help restore proper movement and function to the joints of our spine and body.

Adjustments can enhance the communication 🗣 between our nervous system 🧠 and the muscles surrounding that joint leading to a decrease in tension and pain. Adjustments are typically applied by hand 👐🏻by bring a joint to its end range of motion and applying a high-velocity, low-amplitude thrust.

Sometimes a “popping” sound is heard💥. This is called a CAVITATION and is due to the release of pressure in the joint in the form of an air bubble (a process known as “tribonucleation”🤓). I like to equate this to cracking your knuckles or opening a can of pop!

Chiropractors decide where to adjust, by locating joint tension through a process called motion palpation. This process involves gently assessing restricted movement of joints with our hands.

Don’t want to be adjusted? No problem!
Chiropractors can utilize many different techniques to help relieve your pain and get you moving again. These include:
✨joint mobilizations,
✨muscle release techniques,
✨muscle stimulation,
✨acupuncture and,
✨therapeutic exercise

While adjustment are safe ✔️ when applied by a train professional, they are not always warranted for every condition or every individual.

Today, on Rowan’s Law Day, we remember Rowan Stringer, a high school rugby player from Ottawa, who died in 2013 from a condition known as second impact syndrome. It’s believed Rowan experienced three concussions over six days while playing rugby. She didn’t know her brain needed time to heal. Neither did her parents, teachers or coaches.

In response, Bill 193 Rowan’s Law (Concussion Safety) was passed in 2018 and sets mandatory regulations for preventing and managing concussions in amateur sports. However, you don’t have to be an athlete to suffer from a concussion. Any substantial blow to the head/neck/body or quick jolt can result in a concussion. Trauma to the head should always be checked out by a health care professional who is trained in concussion diagnosis and treatment.

Chiropractors support Rowan’s Law and are also qualified to play timely roles within it. Chiropractors can assess, provide an informed referral for further evaluation/treatment and diagnose post-concussion symptoms at the field level. They can also treat your neurological spine, muscle and joint-related symptoms and work with your interprofessional care team to help manage your condition.

✨What can I expect at my first chiropractic appointment?✨

Great question!

Not everyone knows what to expect on their first visit to a new healthcare professional 👩🏻‍⚕️, and because of this we know that you may need some extra time in your first appointment.

Depending on the injury or condition, initial assessments can range from 30-60 minutes ⏰This ensures there is plenty of time to get a good understanding of your complaint, and discuss a proposed plan of management.

1️⃣HEALTH HISTORY: a thorough health history involving not only your area of complaint and the surrounding joints, but also your personal and family medical history.

2️⃣FUNCTIONAL EXAM: the purpose of this exam is to try to reproduce your pain to better understand its root cause, and to see your body mechanics. Have no fear! Your chiropractor will talk you through the entire process so you are well informed.

3️⃣REPORT OF FINDINGS: once the exam is complete, your chiropractor will explain to you their findings and diagnosis. We can answer questions, address concerns and explain the different treatment options for your condition. This may include preventative strategies, changes in lifestyle practices, exercise prescription or even referrals to other health practitioners.

Similar to the discomfort you may feel after a workout, you may feel some slight soreness or fatigue after your first visit. No worries, this is totally normal and should resolve quickly!

4️⃣Treatment: depending on your condition, your chiropractor may or may not provide hand on treatment the first day. If an adjustment or manual treatment is something you feel would benefit you, let you chiropractor know so they understand your expectations and can help you feel better!

Open communication is KEY 🔑 to maximizing the benefits of your chiropractic treatment; let you chiropractor know how you are feeling and if there are any changes you may need in your subsequent visits!

Bottomline: your chiropractor is there to help support you in achieving YOUR health goals! You are in control of your health, and we are here to help guide and support you in living the most functional, and healthy life possible.

“Backpack, backpack” 🎶

Does anyone else sing the Dora the Explorer song when they put on their backpack? No? Just me? Okay, cool. 😂

❓DID YOU KNOW❓the recommended weight for children’s backpacks is 10-15% of their body weight? Interestingly, some studies have noted children carrying up to 46% of their total bodyweight! 😯

The highest rate of growth is between the ages of 10-12 for females, and 13-15 for males. Ironically, these tiny humans are the ones most vulnerable to developing an overuse syndrome as their muscles and bones have not fully developed, and are more at risk to microtrauma. Because of this, proper backpack etiquette should be emphasized during this time 🎒

Here are some key things to look for when assessing the quality of a backpack to help avoid fatigue, and maintain a happy and healthy back:

✨A padded back to reduce pressure and enhance comfort
✨Contoured and padded shoulder straps to reduce pressure on the shoulders and permit free movement of the arms. 
✨Hip and chest belts should be present to transfer some of the backpack weight from the back and shoulders to the hips and torso
✨Multiple compartments to better distribute the weight, keep items secure, and enhance access to the contents
✨Compression straps to stabilize articles and keep the contents of the bag as close to the back as possible
✨Reflective material to enhance visibility to drivers at night

Lastly, make sure kids are getting their recommended 60 minutes of moderate to vigorous physical play per day! This will help maintain muscle and bone strength necessary for carrying that school bag💪🏼🤸🏽‍♂️

Where’s my WFH people at?! 🙋🏻‍♀️

While waiting for licensure, I’ve recently taken on a full-time job that requires me to work from home (WFH) at a desk. If I’m being honest, it has genuinely been challenging for me both mentally and physically! 🥵

Whether you are working, or attending virtual class, here are my some of my tips for improving your workstation, staying positive, and maintaining productivity while at home:

1. ENVIRONMENT: Surround yourself with things that make you happy! For me that’s plants and pictures of my friends and fam

2. SET A TIMER ⏱: I’ve only just discovered how great the “Hey, Siri” function is on my phone. Setting timers help to create small attainable goals during my workday, and it gives me time to stand and move.

3. ADJUST YOUR CHAIR🪑 An ideal chair allows for your thighs to be parallel with the floor with your knees in line, or slightly lower than your hips chair.

4. MONITOR 💻 The top of your monitor should be at eye level to keep you in neutral position. HACK: If your monitor is not adjustable, stack some books to raise it up a little!

5. LIGHTING💡 Being near natural light is great to soak up that vitamin D, but move monitor perpendicular to the window to avoid glare and eye strain 👁

6. KEYBOARD ⌨️ Your keyboard should be in a place that allows you to rest your elbows at your sides so your forearms are parallel to the floor. If you use a laptop regularly, consider investing in a wireless keyboard and mouse. I found this super helpful in keeping me comfortable for longer!

7. KEEP MOVING! 🤸🏽‍♂️Maintaining good posture can be challenging, so don’t stress if your posture isn’t “perfect” 100% of the time. The key to maintaining good posture is by changing it often. Try to change your position every 30-50mins and take a little break to stand and stretch.
