🩺 understanding Cholangitis & Cholecystitis Explained 🩺

Cholangitis: Inflammation of the bile duct system in the liver, transporting bile from the liver/gallbladder to the duodenum. Bile aids in digesting fats and cholesterol. Causes include bacterial infections or autoimmune conditions.

Cholecystitis: Inflammation of the gallbladder due to blockage by gallstones or bile sludge. Gallstones, made of cholesterol, affect nearly 1 in 10 adults.

Types of Cholecystitis:

Calculous: Blocked cystic duct by stones/sludge, increasing pressure and causing inflammation.
Acalculous: Inflammation without stones, due to severe illness, surgery, or trauma.
Chronic: Repeated acute attacks treated conservatively, leading to gallbladder scarring.
Risk Factors for Gallstones: Overweight, female, fertile, fair-skinned, age 40+, family history.

Key Terms:
Cholecystolithiasis: Gallstones in the gallbladder.
Choledocholithiasis: Gallstones in the bile duct system.
Factors Increasing Gallstone Formation:
Disruption of enterohepatic circulation (e.g., Crohn’s disease).
Fasting, rapid weight loss, parenteral nutrition, bariatric surgery.
Haemolytic anaemia, increasing bilirubin.

Endoscopic ultrasound: Detect small gallstones.
ERCP/MRCP: Diagnostic & therapeutic (ERCP), diagnostic only (MRCP).
CT-Scan: Operation planning.
Abdominal X-Ray: Suspected gallbladder tear.Treatment:
Pain & cramping medications.
Fasting to relieve gallbladder strain.
IV fluids to prevent dehydration.
Antibiotics for high inflammation.
Surgery to remove gallbladder for acute cholecystitis prevention.
ERCP for therapeutic interventions.

Causes of pneumonia vary and include bacteria, viruses, and fungi. Risk factors encompass age, seasonal changes, and pre-existing conditions like asthma or COPD.

Types of pneumonia:
🏠 Community-acquired pneumonia: Common bacteria include pneumococcal and Haemophilus influenza, along with viruses like Influenza and RSV.
🏥 Hospital-acquired pneumonia: Often caused by bacteria like Pseudomonas aeruginosa and Staphylococcal.
🛌 Pneumonia in immunocompromised patients: Typically due to minor bacteria and fungi.

Tears of an 11 year old“ 😢

Today was my first day on leave so I thought, I should work a bit on our Website. The whole family was at home relaxing and reflecting on how tough this year was, especially for families. 👨‍👩‍👧‍👦 After a while my eldest son came sat next to me curiously observing what I was upto. 💻For some reason he got so upset when he saw this page on our donation site ,,Dokis-advice.com/support-us“ . He asked me with so much Compassion : Dad, you only have Zero Donations raised? But Why? That’s all I heard before he rushed upstairs with his wet eyes. A few minutes later he comes down shedding tears 😭 with his hands open and 40 Euros folded from his own savings. Which is a lot of money. 💶He murmured ,, here is my donation,, now you have 40 / 100000 Euros 😳 for building your new hospital in Kenya. 🙆🏾‍♂️I was so touched that I didn’t know what to say or how to react. I tried explaining to him that we have had several well wishers, but he insisted I take his donation. If this would be the positive reaction from everyone who visits my page, the project would have been finished by this year. What my 11 year old son didn’t know is he just placed the first stone on that future hospital building. And for those who read the bible, 📚I don’t think it was a coincidental figure of 40😇 So Dear God, thank you for making it your cause. 🙂

Sie lag nach der OP eine Woche auf der Intensivstation. Sie hatte Krampfanfälle und konnte nicht sprechen.
Das ganze Team von Doki's Advice hat sich große Sorgen um sie gemacht.
Aber jetzt kann sie wieder laufen und sprechen, hat wieder die Kontrolle über ihre Mimik.

Das war ein wirklich großartiger Tag für Stacy und ihre Familie - und für unser Team 🍀 - wir feiern ihn!

Attention!

We have had 2 camps running at the same time, the surgical and the internal Medicine one.
The internal Medicine Camp closed on the 24th.Nov
The Surgical one closes on the 28th.Nov.The Theater Plan is therefore already finalized and due to Limited Time, no further Patienr reviews take place.

We know the Flyer states that the Camps run until 30.11. One of our Surgeons who was to carry on till 30th could not make it due to unavoidable circumtances.

We apologize for the inconvinience, since most patients cover quite a distance to Kisumu.

Next Neurosurgical camp takes place in Feb 2024.

Jetzt ist die Zeit gekommen für Doki's "Workation", das ist Doki's work-and-vacation-Programm. Zuerst wird gearbeitet, dann Urlaub gemacht.
Nach zwei Wochen harter Arbeit, geht jetzt der entspannte Teil der Reise los. Dieses Jahr gab es das erste Mal keine Todesfälle bei der Arbeit im Krankenhaus. Das Team hat aus den Fehlern gelernt, Prozesse wurden verändert und durch mehr Personal konnte ein gewisser Standard angeboten werden. Insgesamt 21 Menschen aus dem medizinischen Bereich sind aus Deutschland angereist und haben gemeinsam mit den Ärzten und Krankenschwestern in Kenia gearbeitet.
Jetzt genießt das Team miteinander die letzten beiden Tage und freut sich aufs nächste Jahr. Gemeinsam überlegen und planen sie schon, wie sie es noch besser machen können.

Dies hier ist auch ein Aufruf an alle Krankenschwestern, Ärzte, Notfallsanitäter, Physiotherapeuten und Anästhesisten... Wenn du dich gerne ehrenamtlich engagieren möchtest in einem Krankenhaus in Kenia und dies mit einem Urlaub verknüpfen, dann melde dich bei uns. Im Anschluss an das Camp wirst du für deine Mühen mit einem einzigartigen Kulturangebot belohnt und einem super Urlaub in toller Gemeinschaft, organisiert von uns.

Das Team von Doki's Advice war auf einem Tagesausflug im Nationalpark unterwegs. Schwerer Regenfällen haben die Straßen in eine matschige, rutschige Landschaft verwandelt.
So war der Tagesausflug eigentlich nicht geplant gewesen, aber bei dieser Aktion konnten sich alle noch mal ein bisschen besser kennen lernen. Sie mussten als Team miteinander anpacken und sich gegenseitig helfen.
Es war ein richtiges Abenteuer.
Das Team genießt die letzten Tage hier am Viktoriasee und hofft, dass im nächsten Jahr noch mehr Helfer dabei sind.

Hier auf dem Foto seht ihr Dr. Gerald Otieno mit der Ehefrau eines Patienten.
Das Team von Doki's Advice war auf dem Markt einkaufen, als diese Frau ihnen Bananen und Orangen anbot. Plötzlich erkannte sie Daniel Ogutu "Oh, you are Dr. Dan". Das besondere an dieser Geschichte ist, dass der Markt, auf dem sie sich getroffen haben, über eine Stunde vom Krankenhaus in Lwak entfernt ist. Ihr Mann war so schwer erkrankt, dass er drei Tage am Stück von den Ärzten unseres Teams behandelt werden musste. Und er hat einen so weiten Weg auf sich genommen um Hilfe zu bekommen.
Die Frau ist einfach überglücklich den Helfern noch einmal begegnet zu sein 💗
