Hey y’all! Took a few days off for vacation but now I’m back! 🌊 ☀️ ⁣⁣⁣⁣ ⁣⁣⁣⁣ Here’s a series of OCTs, but we’re not focusing on the retina. Today, it’s all⁣⁣⁣⁣ about the choroid! Here are some tips on how to spot a thin or thick choroid on the OCT:⁣⁣⁣ ⁣⁣⁣⁣ 𝐓𝐡𝐞 𝐛𝐚𝐬𝐢𝐜𝐬: the choroid consists of 5 layers: Bruch’s Membrane⁣, Choriocapillaris, ⁣⁣⁣ Sattler's Layer (medium diameter blood vessels)⁣, Haller´s Layer (large diameter blood vessels)⁣ and the suprachoroid (where the choroid transitions to the sclera)⁣. The entire choroid is on average, 260 microns thick but becomes thinner with age. See photo 3 for a normal OCT of the choroid. ⁣⁣⁣ ⁣⁣⁣ 𝐓𝐡𝐢𝐧 𝐜𝐡𝐨𝐫𝐨𝐢𝐝 (photo 1): this is an example of what a thin choroid looks like on an OCT. The pathology here is Myopic Degeneration (this one with retinal fluid, yikes!), which classically, consists of a thin choroid. What are you really seeing? Mostly sclera, which is why it’s mostly hyperreflective below the retina. The white arrow is the thin slice of the almost non-existent choroid. ⁣⁣⁣ ⁣⁣⁣ 𝐓𝐡𝐢𝐜𝐤 𝐜𝐡𝐨𝐫𝐨𝐢𝐝 (photo 2): You’ll see a thick choroid most commonly in a spectrum of choroidal diseases known as the pachychoroid spectrum. I won’t go much into the diseases that are in this spectrum on this post (you can write an entire book on them!) but a common one is central serous chorioretinopathy. On this OCT, note the overall larger layer of the choroid (use the OCT’s measuring tool if you need to).
