🌟 A Happy Journey to Menopause Management 🌟

👩‍⚕️👨‍⚕️We recently had the pleasure of assisting a vibrant woman in her mid-50s at Infiniti Healthcare, seeking support for managing her menopause symptoms. Here's her uplifting story:

👩‍⚕️After a total hysterectomy in her 40s, she experienced surgical menopause and started on Evorel 50 patches. During her consultation, we used the Green Climacteric Scale to evaluate her symptoms, scoring 23 out of 63, with severe headaches and muscle/joint pains being the most challenging.

🩺Working collaboratively, we reviewed her lifestyle, emphasizing stress-reduction strategies. Hormone blood tests revealed low oestradiol and testosterone levels, prompting a tailored plan to optimize her hormone therapy.

🩺She was eager to start Testosterone therapy, but first, we increased her Evorel 50 to 75 for 4 weeks before introducing Androfemme. After 10 weeks, her oestradiol levels increased significantly, and her SHBG dropped, although her testosterone level remained unchanged. Most importantly, she felt fantastic, with her symptom score dropping to 8 out of 63!

🎉 The takeaway: Blood tests aren't always necessary for managing menopause and HRT, but they can provide valuable insights to guide and monitor treatment.

Meet Sue: Sue is a vibrant 75-year-old who recently visited Dr. Nadia with symptoms of pelvic organ prolapse. These symptoms were so bothersome that they kept her from enjoying her favorite activities, like dancing.

Understanding Pelvic Organ Prolapse (POP): POP occurs when one or more of the va**nal walls or the womb start to bulge. This typically results from a weakness in the pelvic floor, often due to childbirth.

Dr. Nadia's Approach: Dr. Nadia began by taking a detailed history to understand Sue's symptoms and how they were affecting her daily life. She learned that Sue cherished dancing and walking, but her condition was significantly limiting her ability to enjoy these simple pleasures.

The Treatment Plan: After a comprehensive assessment and examination, Sue decided to try a va**nal ring pessary. In addition to the pessary, Dr. Nadia prescribed va**nal estrogen cream to aid in her recovery.

What is a Vaginal Ring Pessary?: Vaginal ring pessaries are simple, versatile devices made from materials like PVC or silicone. They come in various sizes and shapes, and it can take up to six trials to find the most comfortable fit—much like finding the perfect pair of jeans! 😊

A Happy Outcome: Sue had a wonderfully successful experience with the va**nal ring pessary. She is thrilled to be back on the dance floor and enjoying life to the fullest once again!

Up to half of all women experience weakness in both the abdominal muscles and pelvic floor muscles after pregnancy and up to a third still have a tummy gap at eight weeks post-birth. The intiial assessment is an hour long appointment and includes a full postural screen, a pelvic floor muscle assessment, an abdominal muscle exam to check for muscle separation and screening for bladder, bowel and s*xual dysfunction. The Uncommon Story of Painful S*x

I wanted to share something this evening that I think all people should know.

Painful s*x, is common at most key stages of women’s lives.

It starts with when we start having s*x, with gynaecological conditions such as endometriosis, after having a baby, following scarring at the entrance of the va**na, trauma, forced s*x, during the perimenopause, and after the menopause.

Painful s*x is commonly attributed to being a female problem.
“The va**na is too dry- have some creams…etc”
“ You’re too tense- you need to breathe, relax”
“ You have vaginismus - you need botox in the pelvic floor”
And the list goes on… but how many health care professionals ask you about your partner, their health and more importantly how they perform ?

A big question is

“ Does he stuff ? Or does he pe*****te ?”
“Is he soft at the start or is he hard ? “

Not many people talk about the soft and flaccid p***s which is a very common cause for painful s*x.

I ask this question whenever I see a patient with painful s*x… and it has saved some marriages !

It gets women thinking … it’s not all her fault ! … and may not be due to her at all…

This question generates thoughts, nudges couples to talk, to be honest and face facts … and for some men taking action, finding the cause that may not just help their relationship but save their life 🙏🏻

***Advanced Vaginal Pessary Masterclass AVPM ***

I wish to announce that we are running our 2nd AVPM in May 2024 with a few spaces available 🥳

This masterclass will be invaluable for Pelvic Health Physiotherapists, UroGynaecology nurses, Gynaecology nurses aspiring to apply for UroGynaecology positions and doctors working in the field of pelvic floor health.

The AVPM is a one of a kind training programme. Delivered by expert Consultant UroGynaecologist Kiron Bhal & myself, with London based, Tracey Mathews, expert Pelvic Health Physiotherapist running a highly successful private va**nal pessary service.

The theory sessions cover basic and practical knowledge with shop-floor golden nuggets based on UK clinical guidance and logbook. The workshops and hands on training is delivered face-to-face with Clinical and Patient Teaching Associates covering everything from abdominal and pelvic health assessments, speculum examination, cube and ring sizing and fitting and so much more ☺️

Following feedback and multiple requests we now offer two options
A) Theory only - provided online with CPD certified certification

B) Theory ( online) & Practical ( face to face) with formal Zoom reflection catch ups.

All delegates will be invited to a Facebook members only group.

Full masterclass ( theory and practical) delegates will be invited to the Facebook members page as well as a private WhatsApp group for ongoing mentorship and support.

We would like to welcome Melissa Jenkins to the Infiniti Healthcare Team. Melissa is a qualified Physiotherapist who specialises in Womens Health, Pain Management and Trauma. Melissa is passionate about pelvic health which stems from her own experiences of pregnancy and postpartum recovery following having her two children. She is excited to be able to deliver detailed postnatal physiotherapy assessment of the abdominal and pelvic area for women following both va**nal and caesarean deliveries.
