Keratosis obturans (KO) is a rare condition of the ear canal defined by accumulation of keratin that leads to dilation and blockage of the ear.
Sometimes it’s less obvious, but in today’s patient the widening was very obvious and I could feel bone where the erosion was. The sound and feel of metal on bone is very distinctive!

Crunchy wax 🥴 Before and after!

On first look the drum looked clear, but my lovely patient was experiencing muffled hearing. A little spray of sodium bicarbonate helped to reveal a layer of skin over the ear drum that hadn’t migrated as it should. A gentle hover with the small suction tube: skin gone, hearing restored!

If you have an outer ear infection (otitis externa) that won’t clear, get in touch. If your ear is full of infected matter it’s hard for the treatment to get to the right part, but clearing it with microsuction allows treatment to get to the skin underneath making it much more effective.

My Loops got me through the night as we currently have a wasps nest in our inaccessible roof space above the bed. They make a lot of noise, buzzing and popping (like a dripping tap) all night.
If you need a bit - or a lot - of noise cancelling, look for Loops
I’m told the wasps should settle down in October😱😱😱

My current portable suction unit doesn’t owe me anything, it’s been an absolute workhorse and is still going strong.
Not to tempt fate, but I’ve brought a replacement, just in case!

Although I remain a qualified nurse with 24 years experience in both acute and community settings, I am no longer registered with the NMC (Nursing & Midwifery Council). This is due to a legal responsibility - it is currently illegal for registered nurses to carry out ear wax removal unless they are registered with CQC (Care Quality Commission). This does not change the care I give in anyway and my knowledge, experience and skills remain unaltered, as does my full and comprehensive insurance, it just means I’m working within the law 🙂

There are lots of positives to being self employed. This is not one of them😂

The manufacturers of Otex say that Hydrogen Peroxide helps to soften hardened earwax which then breaks down into small pieces and falls from your ear.

What will using too much hydrogen peroxide do to my ear?

Too much of the chemical can cause blistering and skin irritation. So if the wax doesn’t come out you are left with a chemical infused wax plug sitting against your delicate skin.

Are there any other eardrops that are safe to use?

It is better to use a gentle formula. Eardrops like Earol or Sodium Bicarbonate drops are much less harsh than hydrogen peroxide.

The first and third pictures show very tender ear where Otex was used. The other pictures shows a much happier ears two weeks later.

The BBC News (when the newsreader is presenting from a desk!) is a good indicator. If your volume button is going up to hear this it might be time from for a hearing test.

It took all the tools to liberate this one!

I’ve been mastering the art of these little instruments.
Sometimes microsuction or warm water irrigation aren’t enough to clear a very full ear so clinicians might use specially designed tools.
These angled ear picks and curettes are new to the market and very useful in a bendy ear.

I guess I’ve treated 6000+ ears, but I’m still learning, still progressing 😊
