Stage Two Hamstring Rehabilitation
During stage two of hamstring rehabilitation, the primary goals are to continue to improve the mobility, strength & motor control, and function while gradually increasing the load and controlled tempo of exercises to continue to gradually increase tolerance capacity, building the hamstrings to be more resilient. Therefore continued development towards stage 3 Rehabilitation
•Elevated Glute Bridge
Perform 2-3 sets of 10-15 repetitions.
•Banded Isometric Hamstring Curl
Hold the in a 15/20 degree flexed position for building up to a 45 seconds hold.
•Progressive Banded Isometric hamstring curl
Increase the knee flexion to 45 degrees and create an isometric hold.
Again whilst building up to a 45 second hold.
•Glute and Hamstring Development Machine
Perform 2-3 sets of 10-15 repetitions, gradually increasing the resistance as tolerated.
•Introduction to Dual-Legged RDL Pattern
Start with light weights and 2-3 sets of 8-10 repetitions, gradually increasing the load.
Regular Assessment:
•At SWO clinic we will continue to monitor your progress and adjust the program under the our supervision. As well as using VALD technology to test for any asymptomatic pattens which we can address early on.
•This will ensure proper form and technique during all exercises with analytic data to back it up.
•Using the force deck and based on your subjective feedback we can progress at a pace that allows for steady improvement without causing pain or discomfort.

Navy Rugby 2023 season
What a few days it’s been! I have been very fortunate to have been working with the Navy over the last week. As part of the navy’s medical team I have been assisting the team in preparation for this weekend game, Army VS Navy at Twickenham!
I will be looking forward to working more with the Navy next season!

Part 1 Planter Heel pain
•Symptoms are typically sharp and normally present after getting up in the morning, Or after prolonged standing or from returning to stand after being seated for a period of time.
🌍 10% of the population suffer from planter heel pain. It’s more common in runners and overweight individuals and can be associated with other factors such as arch type, the overall strength and the mobility of the ankle and foot.
•Loading mechanics 🔧
Tendons respond to loading by an upregulation of insulin-like growth factor ⏩ remodelling within the tendon Strain should exceed that of ADL to trigger adaptation Basically, loading leads to improved tissue health.
•Should you still stretch? There are suitable stretches for conditions such as plantar fasciitis. We would say active passive movements for the foot and ankle would be beneficial. You can see a previous on Plantar Fasciitis post to see them being demonstrated.
•In the next post we will be comparing different types of exercises you can use, and explore the latest evidence on additional treatment options such as extracorporeal shockwave therapy (EWST), manual therapy, surgery, and corticosteroid injections.

💥 Meniscus Tear 💥
The Knee joint contains two compartments a Medial and lateral meniscus. The medial and lateral meniscus acts as a shock absorption and force transduction. Injuries to the menisci can be traumatic or degenerative in nature. These injuries are more common in athletes, where they are twisting/rotating on a panted foot during a performed movement in sports such as football, Rugby, Skiing! And Cricket etc…
🔍Symptoms associated with meniscus injuries are usually reported a general knee pain, with catching, locking or even buckling of the knee, and sometimes a combination of all three.
🚑 What is the most effective approach when it come to meniscus injuries? Well, NOT all meniscus injuries require surgery!! Surgery is normally indicated when a mechanical symptoms such as locking. Otherwise a conservative management approach would be highly recommended such as physical therapy. Check out tomorrow’s post on Arthroscopic Partial Meniscectomy vs Physical Therapy for Traumatic Meniscal Tears in a Young Study Population. This will be a follow up from this post. The article reviewing is a RCT study which was recently published early this year.

Arthroscopic Partial Meniscectomy vs Physical Therapy for Traumatic Meniscal Tears in a Young Study Population: what does a recent RCT show?
Following on from a previous post. In this study it has provided the data to suggest that the evidence from studies on older patients with degenerative meniscal tears showed that surgical treatment has no benefit compared to non-operative treatment, but there are no high level studies that compared these treatments in younger patients with traumatic meniscal tears.
What did the study find?
We included 100 patients, 49 patients were randomised to arthroscopic partial meniscectomy and 51 patients to physical therapy. In the physical therapy group 21 patients (41%) underwent a delayed arthroscopic partial meniscectomy during the follow-up period of 24 months because of persistent complaints. Both groups improved in IKDC score during the follow-up period of 24 months. At 24 months we did not find that arthroscopic partial meniscectomy is superior to physical therapy in IKDC score at 24 months (between group difference 0.1, 95% confidence interval -7.6 to 7.7; p-value 0.99).
What are the key take-home points?
Young patients (aged 18-45 years) with traumatic meniscal tears who were treated with arthroscopic partial meniscectomy, compared with those who had physical therapy plus optional delayed arthroscopic partial meniscectomy, had similar perceptions of symptoms, knee function, and ability to participate in sports at 24-months follow-up. 59% of the patients randomised to physical therapy did not undergo delayed arthroscopic partial meniscectomy during the follow-up period. Results from this study suggest that there is no need to consider arthroscopic partial meniscectomy to be the first-line treatment in patients with a traumatic meniscal tear and physical therapy with optional delayed arthroscopic partial meniscectomy may instead be appropriate.

Occlusion training
How does Occlusion training work?
Under normal conditions slow twitch muscle fibres are recruited first and as intensity increases fast twitch fibers are recruited. Occlusion training causes a restriction in blood flow so the fast twitch fibres are recruited at a lower intensity. Fast twitch fibres have a higher capacity for hypertrophy. It has also been shown to cause a metabolic build up producing a positive physiological reaction, specifically a significant increase in growth hormone.
In the clinic we have utilised occlusion training for clients following a knee arthroscopy, ACL reconstruction and post Achilles repairs and Ehlers-Danlos syndrome. Helping to build muscle without putting excessive strain on the joints, soft tissue and other structures or the surgical repairs.
We have also used this method for athletes in sport to increase there capacity and create positive adaptive change whilst causing little or no delayed onset muscle soreness (DOMS).
