I was quite interested in this decision published this week by Richard Turfitt.

For me, it shows that no matter what exemption you may be operating under, you are in no way exempt from the obligation to keep your vehicles, people or the wider general public safe from the risks associated with being an operator. Thankfully in this wheel loss case there was no passengers on the vehicle or this could have been a very different case.

If you operate any vehicles, no matter how big or small or the size of the fleet you have a duty of care and a moral obligation to ensure they are fit and serviceable at all times. If you are unsure if you are doing things right, seek appropriate professional support. Don't just wing it with someone's life, ask yourself would you be comfortable with your family driving alongside the vehicles you operate on the motorway ? If the answer is no, then fix the problems before it is too late.

The only silly question is the one that doesn't get asked.

Did you know the most common month of the year for bridge strikes is July ? Do you just put an agency or temporary driver in the vehicle or do you have a full induction and training process with them ?

Better to spend a day training and inducting drivers than to spend a day with the Traffic Commissioner explaining what went wrong.
