Our lovely patient had a new combo treatment on Friday - we combined two of our favourites, and to synergise and give a more comprehensive multilayered skin rejuvenation.
The numerous “sandy” specks are actually little columns of extruded dead tissue called “MENDs” or Microscopic Epidermal Necrotic Debris for long.
The individual wounds stimulate a healing response which results in newer, fresher, healthier skin.
This patient had a strong one too, so we can’t wait to see her evolution!

This lovely lady travelled to see us for acne scar treatment.
Following a combination of subcision and (fully ablative), she was very pleased.
Things are settling down nicely. A little redness and pigmentation will slowly settle with continuing skincare.
It’s great when the team gets to make a difference.

Scar following lesion removal lower leg.
6 weeks out.

Suture-mark scars will be due, in part to suture tension - this was unfortunately necessary, to some degree, to avoid dehiscence/wound separation.

New scar tissue has plentiful blood-supply which facilitates scar growth. Reducing this with ‘vascular’ wavelength light-therapy has been shown to close down this excessive vascularity, improving colour, texture and pliability of scars.

Here BBL at 560nm has had an immediate effect on scar redness, helping to alter the healing trajectory of the scar.

Life-changing results don’t happen because of one person.

And isn’t it better to be part of a team anyway?

This patient had a very severe rhinophyma.

Now he doesn’t..

It took a journey involving 2 treatments across approximately 5.5hrs of laser time.

It was preceded by the usual checks and a pre-treatment “powwow” with to brainstorm the best approach. He actually offered to help but geography was challenging.

After the second treatment, healing was delayed by an infection and the wonderful (the amazing Astra who is a fellow .collaborative expert and tissue-viability guru) provided wound-management and dressings support and guidance.

The team provided continued support and kept our lovely patient smiling.

And now he feels like a new man.

Described by some as life changing..
This gentleman certainly found his rhinophyma distressing and affected his quality of life to a significant degree.
A single treatment utilising our ultrapulse laser has certainly brought balance to his visage.
Never resting on laurels, we always seek ways to maximise outcomes and recoveries.

These 3 wild ones went to the market.
En route they were criticised for not riding the donkey, for the boy, then the old man, then them both riding the donkey..
It seemed they couldn’t please everyone.
So the boy and the old man fashioned a sling and made to carry the donkey.
But the donkey kicked, fell in the river and drowned.

The moral of the story is don’t try to please everyone. You can’t.
And why would you even try?

In life we should chart our own path and choose those you want to share the journey with.
The rest will find other routes. And good luck to them 🍀

Keep moving forward.
But remember the past.
Doff your cap to the fallen.
And make them proud when you can.

And even if you’re a slow-ass, grind it out. Because you never lose until you give up.

Does acne scarring matter?

This interesting study helps provide some perspective..

The graph enumerates the first thing noted about a person’s face. This when viewing patient picture with either digitally superimposed acne scars or with clear skin. [Dreno et al 2016].

When acne scarring is present, the skin is more noteworthy. And not in a good way.

Thankfully, we have ways of effectively improving scars affecting real patients.

Predictions are that this year, in Scotland, it may be on a Tuesday..

The skin-health benefits of sun-protection are beyond debate.
But sometimes we think that just grabbing a hat will be less trouble than applying cream.

Sadly, most hats offer little protection for the face. The best results in an Australian trial for non-forehead areas were protection factors of 8-10.
Even wide-brimmed hats don’t provide the anticipated protection from the highly reflective, short wavelength UV rays.

As with most things in medicine, a combination approach works best.

This is definitely not the inside of a person.
This here is some spaghetti sauce!

I read with interest that for some 30 years, focus groups were asked what they wanted from their spaghetti sauce.

Turns out 1/3 want sauce with bit in it as opposed to homogeneous liquid sauce.

Yet in 30 years, nobody volunteered or was consciously aware of this fact.

So it is with patients and the expected outcomes. Patients often don’t have a clear vision of what they want or can realistically be achieved.

The consultation process is crucial to ensuring we’re on the same page.

The process isn’t just the in-clinic assessment, rather it includes the information provided and the dialogue up until the point of carrying out a procedure.

Of course we can always improve this process, and we never stop pushing for those incremental improvements.

Meanwhile I’ll keep searching for the ideal tangy, lumpy, low-basil sauce..

If it makes a difference..

Patients seek results, like in this case of significant sun-damage and deep perioral lines.
The treatment isn’t for the faint hearted and they are thorough counselled beforehand and supported throughout.
We are honest with patients and they know they are on a journey rather than a TV show.
Additional treatment is necessary to manage the over-zealous healing of her right upper lip. But that’s OK as we’re in it together.
And she sure as hell has seen a dramatic difference already.

Make-up.

Not something I make use of personally. Ok maybe on special occasions..

But my patients like it more than I do and they want it to sit nicely on their skin. And they especially do not make their skin look worse!

This lovely lady had fully ablative laser resurfacing for acne scarring.
Both photos have make-up. Not something I recommend for comparisons, but the patient finds it very important. And is VERY PLEASED that her makeup is now sitting better.

I’m never completely satisfied with any treatment outcome, but here I’m quite pleased.

A work colleague is excited to have tickets to see soon.

I love their song titled ‘dignity’, and the line ‘And he takes no lip off nobody’.

You could argue that we take lips off people, however we replace them with newer, fresher ones.

And while we may not restore or preserve dignity, we do restore confidence and freshness.

This patient was troubled by severe and was looking for a solution.

As you can see, ablative laser provides the opportunity to address the problem.
Here we used ErbiumYAG and CO2 lasers in combination to remove the glandular hyperplasia.
One treatment and we’ve made significant progress.. yes I’m incredibly hard to please but that’s why we get good results.
