Responsive Care Practitioner

Imagine this. You have been happily living independently for many years, however one day you have a medical emergency. You spend some time in hospital, and are then discharged home, but life is now different. The independence you once had feels like it has disappeared. You are aware that you can no longer do certain tasks in your day-to-day life, you know you need support, but accepting help can sometimes be difficult.
You decide that it is for the best to accept this, but are still wary, until you meet your new Responsive Care Practitioner. They work with you to start living the new normal, they support you with starting the journey to regain your confidence, they always greet you with a warm smile and a cheerful hello. After a couple of weeks, you start feeling more positive every day.
This could well be the mindset of one of your clients as a Responsive Care Practitioner, and it will be your job to provide care on a short-term basis to those who are going through a change in their lives. That's a wrap on this years work experience placements!
We've hosted seven students over the last few months, ranging from Year 10 and 11 school students, Year 12 and 13 sixth form and college students, and degree students.
Each student has been out to visit and help out at different services within the adult social care sector in North Somerset.
It's been fantastic to give students the opportunity to see just how varied the sector is.

Having the chance to inspire the younger generation who have shown an interest in the Health & Social Care sector is really important to us.

Our responders are a specialist team working across North Somerset, attending emergency callouts for our clients. The role is varied, and no two callouts are the same. This is a fast paced, highly rewarding position in a team using cutting edge technology to provide high quality emergency care.
This role will require you to have a full driving license due to the service area.

This role pays block pay, this means from the second you start your shift to the second you finish you will be getting paid, meaning you will know exactly what you will be getting paid for the hours you work. This is something of a rarity in the social care industry.
You will also not have to worry about using your own vehicle to work. Access Your Care provides our response teams with company vehicles whist on shift.

Here at AYC we offer the use of a company electric bike to carers. There are so many benefits of being able to supply the E bikes.

John one of our bike riding Adult Care Workers had this to say about riding:
"Before I started working for AYC I was diagnosed with AF and a few other age related health issues. I was told by the doctor to exercise more, so having the use of the ebike has really helped improve my health, I don't get stuck in traffic jams and I keep my tan topped up too!
It can get a bit cold and wet in the winter but I have a poncho supplied by AYC that may come into use."

Did you know...last year, we started a service to help people with dementia and prevent unnecessary hospital and care home admissions? 💜
Our dementia wrap-around care team works closely with local hospitals, nursing and therapy colleagues, and social workers. It's the first time we've taken referrals for care and support from such a wide range of partners.
Over a six-month period in 2023, we supported 29 people. Of those, 21 people were able to stay at home during times of ill health instead of being admitted to a care home or hospital. Eight people were discharged from the hospital back home, meaning they didn't have to go to a care home.
We've received positive feedback from people who've used our service. Thanks to additional funding, we can continue and improve this wrap-around care, making a significant difference for people living with dementia and their carers 🙏

We are looking for compassionate, highly motivated people with amazing communication skills and who work well under pressure to help us in providing the highest quality care for those living with dementia.
As a Dementia Care Worker, you will be part of a specialist team of care professionals, working with clients who are at different stages, from newly diagnosed to the advanced stages of dementia. You will need to be kind, compassionate, have a lot of patience and a good understanding of dementia and the various stages of it.
You will be providing support to clients, working with many different public and health professionals ensuring that our clients wellbeing is always accounted for. You will have a level of flexibility with call durations based on individual needs and you will have ongoing support from your line managers.
You will be provided with full training and additional dementia training to carry out your duties effectively. This job will involve assisting with personal care.
You will be working shifts between 07:00 and 22:30 based on a rota, weekend work is required, you will be working one in every two weekends.
What we would like from you;
- Previous knowledge and experience in Health & Social Care
- A passion for working with people living with dementia
- Excellent communication skills
- A natural compassionate nature and patience
- Ability to work effectively under pressure
- A full UK driving license and access to your own vehicle
