Edward Francis Small Teaching Hospital
Department of Peadiatrics
Imperforated A**s (Recto vestibular fistula- ARM)

By Dr Cherno S. Jallow (Consultant Paediatric Surgeon) and Team (The Gambia)
She is a 16-year-old Senegalese girl who presented to us a year ago with an ‘Absent a**l opening’ which was present since birth. She passes stool just under the va**na through a tiny hole with no control. As a result, she smells of stool always which caused her to be withdrawn. She did not attend school because of the illness.
When she presented to our hospital, Edward Francis Small Teaching, we saw her at our Surgical outpatient and prepared her for Posterior Sagittal Anorectoplasty – PSARP (creating a new a**s and re**um) after bowel preparation. After the Surgery, the re**um retracted and the wound broke down. As a result, we did a diverting colostomy. The colostomy got retracted which we had to refashion.
After 6 months, we did a redo of the PSARP. Post-op was turbulent as hard stools were passed which seemed to have broken the wound. 4 months later we took her in for examination under anaesthesia and noted that the repair was intact. We eventually, closed the stoma.
Post-op after closure was uneventful. Today is postoperative day 8, she is feeding well, passing stool for the first time through the re**um like any normal person and most importantly she is continent.
Important to note, these were all done free of charge; no financial cost, rather she is usually provided with food and fare by the Paediatric surgery staff members.

This is a quick update on the status of our dialysis machines. Six are now up and running.

Patients who need urgent attention are all sent to Africmed hospital for management at the expense of EFSTH.

The CMD and the team are working very hard to get all the machines back and running.

The Chief Medical Director, Deputy Chief Medical Director and the entire hospital management, would like to express their deepest sympathy to patients with kidney conditions. Some of our machines are down and we are expecting a technician from Senegal today to fix the problem. Meanwhile, measures are in place to send critical cases to Africmed hospital for dialysis. The DCMD who is a Consultant Nephrologist, have assessed all cases and no one is in life threatening situation. The hospital implores your understanding while we normalize the situation.
