Sharing an Interesting Case - Straightforward but Challenging!

Patient Demographics:
High Risk Female in late 60s having Inter-Trochanteric Neck Femur Fracture on right side with short stature and other comorbidities like Diabetes and Hypertension

Goal:
To make patient ambulatory at the earliest and have a stable fixation.

Plan:
Surgical Intervention with minimal surgical time, small Incision and less blood loss under Spinal Anesthesia.

Intraoperative Armaments:

Antegrade Femoral nail - Short PFN as Femur length was less than the length of Regular PFN available.

Retrograde Nail (Enders Nail): Back up plan - Nail insertion from knee and reaching towards hip.

Dynamic Hip Screw: Good option but bigger incision and more blood loss and delayed mobilisation.

PFNA2: Costly.

Challenges Anticipated:
1. Choice of Implant
2. If antegrade nail how negotiate Anterior Femoral bow as anatomy was unusual

Ex*****on:
Antegrade Nail - Short PFN

Challenges Faced:
1. Negotiating Femoral bow
2. Diameter of implant to be used to achieve proper fit
3. Managing both proximal screws to pass through the neck into the head and manage proper length and head shaft angle.

Take home message:
1. Pre operative planning.
3. Always keep all the options ready during the surgery.
4. Even the simplest looking fracture can turn out to be a nightmare so never be over confident.

Similar fractures may have different outcomes in different patients and they do come with their own set of challenges. I was able to come out of this case uneventful and with utmost satisfaction of giving my best to my patient. Our motto should always be to follow the surgical principles and customise accordingly to cater to each and every patient and get the best results.

Primary Total Knee Replacement done in a patient with full thickness cartilage loss in medial compartment with marrow edema and periartcular osteophyte formation. Medial patellar facet involvement with medial meniscus extrusion was also noted on pre operative MRI Study.

Arthroscopy is used to diagnose and treat a wide range of knee problems.

Because the arthroscope and surgical instruments are thin, your surgeon can use very small incisions, rather than the larger incision needed for open surgery. This results in less pain and joint stiffness for patients, and often shortens the time it takes to recover and return to favorite activities.
