Papua New Guinea healthcare challenges

The conditions when she first traveled to PNG in 2011 were less than ideal. “The hospital was in a state of disrepair, such that it was essentially falling down,” she recalls. “The roads around Lae were dreadful – the potholes were so enormous they could swallow whole cars [see photo]. That made it really hard getting around, because it rains all the time in Lae.”

The facilities and environs of Lae were the least of the difficulties. Home to approximately nine million inhabitants, PNG has among the world’s highest rates of cervical cancer, typically diagnosed at advanced stages; patients often presented with fistulas. The country’s HPV vaccination program started up only during the last two years.

“There’s a very poor culture of getting pap smears and even physical examinations, due to a shyness around health issues associated with gynecological areas,” she observes. “They do have women’s wellness centers and they’re encouraging women to get checked for breast cancer among other screenings, but the programs have not really taken off.”

Head-and-neck cancers are also a significant problem, due, putatively, to the chewing of the highly addictive betel nut. Known locally as buai, it is consumed with a mustard stick dipped in powdered lime.

“People chew buai in the side of their mouth, and often in the same place,” Beaufort says. “They say chewing buai causes a head-and-neck cancer in just five years.”
Tuberculosis, malaria and HIV are also widespread in PNG.
Compounding these health concerns, she adds, are a lack of pathology services in PNG that delays results for weeks; equipment installed without service contracts; equipment breakdowns; a country-wide absence of disease registries; and an unreliable

When I first saw this painting, my eyes were drawn to the bloody abdominal incision. C-section, my brain immediately informed me. Next, my gaze shifted up the umbilical cord to the baby being held upside down by Colombian physician, Dr. Jose Ignacio Quevedo, who is depicted here performing the first successful cesarean section in Latin America in 1844. While two men hold down the patient, another is seen assisting Dr. Quevedo with the procedure. Since the dangers of C-section surgery were well known, a priest is present in the case of death of the patient. The only female professional represented in this painting is shown preparing a bowl of sea sponges soaked in a solution of ground mandrake plant – to be used as anesthesia.

This would not be a common scene if you were to walk into a delivery room today. Most of the team, including physicians, nurses, residents, would be female identifying. According to the American Medical Association in 2015, 85% of residents were female in the field of Obstetrics and Gynecology (1). However, only four decades prior to that in 1975, women made up a mere 16% of the total group of residents or physicians in training (2).

How did this field of caring for women’s health and labor change back and forth between the hands of men and women?

Let’s begin with a history of midwives in early modern Europe (1400-1800). Midwives attended to women in labor at their homes and were responsible for the safe delivery of babies. Sometimes, in the case of emergency or delayed labor, they sought the help of barber-surgeons and their instruments. Most commonly, training for midwives involved apprenticeships, with a few options for formal training at schools. However access to advanced knowledge at medical universities were denied for centuries (3).

The Most Common Gynecological Problems

Dysmenorrhea

Suffering from lower back pain or abdominal pain during periods is very common. But, some women go through excruciating pain, interfering with their day-to-day activities. This is known as dysmenorrhea. When the strong contractions occur in the uterus, the oxygen supply may go down, causing dysmenorrhea. It usually gets better as one gets older youageand especially after childbirth.

Ovarian Cysts

Ovarian cysts come in various sizes. Sometimes women live their whole life in a healthy manner without even realizing that they have ovarian cysts. Basically, it is a sac filled with fluid that starts developing on and around the ovaries. These cysts may or may not be tumors. Usually, they dissolve on their own or your doctor may prescribe OCPs or oral contraceptive pills to help them dissolve. However, they should be monitored closely.

Endometriosis

It is a condition where the inner lining of the uterus starts growing outside the walls of the uterus. Commonly, it starts growing on the ovaries or the fallopian tube, however, it can even grow on the cervix, bladder, bowel, or re**um. Nevertheless, even today the exact cause of Endometriosis Treatment is not known. The symptoms include; abdominal cramps, pain while having s*x, bleeding between period, and even digestive problems.

2.2. Abortion:

Although irregular cycles or painful monthly cramps can be common issues affecting women, they are by no means something that women must adjust to as their norm.
