In babies and children with developmental dysplasia (dislocation) of the hip (DDH), the hip joint has not formed normally. The ball is loose in the socket and may be easy to dislocate.In all cases of DDH, the socket (acetabulum) is shallow, meaning that the ball of the thighbone (femur) cannot firmly fit into the socket. Developmental dysplasia of the hip is a condition where the ball and socket joint of the hips does not properly form in babies and young children. When DDH is detected at birth, it can usually be corrected with the use of a harness or brace. If the hip is not dislocated at birth, the condition may not be noticed until the child begins walking. At this time, treatment is more complicated, with less predictable results. A pediatric orthopedic surgeon (a specialist in children's bone conditions) cares for babies and kids with DDH. The goal of care is to get the ball of the hip in the socket and keep it there, so the joint can grow normally. Ritaz was born with a rare condition of hips known as DDH where her hip joints were out of socket and she used to walk with a limp. Her treatment journey brought her to us in India. Ahmed came to India from Kuwait with a very rare bone condition Fibular Hemimelia. The whole of fibula bone was missing with stiff and bent ankle . He was unable to walk with his foot flat on ground. Hip abduction braces are also commonly used immediately following treatments involving a surgery for dislocated hips in children. This time in a brace helps to reintroduce more range of motion to the hips while the hip is growing and becoming more stable. A hip abduction brace is a foam and plastic material.

There are a wide variety of positioning devices available, but the most common type is this type. In addition to this, other braces called fixed abduction braces are commonly used. Other types of braces and harness are available for nonoperative treatment of DDH in Neonates and Infants.

The choice of device depends on the needs of the family and the treatment experience of the doctor. Most doctors recommend full-time wear for 6 months with the brace. However some doctors allow removal for bathing and diaper changes as long as the legs are kept apart to keep the hip’s ball aligned with the socket.

After the hips become stable, the brace is normally worn part time, usually at night, for another 4-6 months.

Infants are able to learn to roll, sit, commando crawl while in the brace, and toddlers can actually walk in the brace. However, crawling on hands and knees is not recommended.

General guidelines for putting a hip abduction brace on your child:
1)Put a fresh diaper and clean pants on your child. Clean the brace if needed.
2)Place your hands under your child’s bottom. Lift his bottom up with your hands. Apply the brace under his bottom. Do not lift your child by his legs.
3)Wrap the foam around your child’s thighs. Secure the fabric fastener straps snugly around his waist and each thigh.
4)Dress your child in a diaper and a pair of thin, stretchy pants or tights. They should not have any zippers or snaps or a thick waistband.

To keep your child’s skin, hips and brace in good condition:
1)Do not exceed the brace-wearing limits prescribed by your child’s doctor or orthotist.
2)Keep the brace away from heat. Heat can melt the foam or plastic and damage the brace. It can also injure your child.

How to clean the brace?
Each day, dampen a washcloth with rubbing alcohol and clean the brace.
Dry as needed before putting the brace back on your child.

Contact your child’s doctor if:
1)You have concerns about the fit of your child's brace. As your child grows, the fit of his brace will change. A new prescription from the doctor will be needed for a new brace.
2)Your child has the following skin problems: Redness that does not fade after 30 minutes, White areas or blisters, Hard skin or calluses
3)You have questions about your child’s brace.

The brace is designed to provide a stable way for the hips to heal. However, the brace may cause problems/ have complications:
1)Cause too much pressure or friction on the skin.
2)Be worn too loosely, preventing the hips from healing properly.
3)Increase sweating.
4)Increase your child’s risk of falling due to the way the brace holds the hips. Have your child learn to walk in the brace in a safe area.

Untreated Cerebral Palsy leads to severe lever arm dysfunction and abnormal twisting in leg bones and feet. A multilevel correction helps to correct the dysfunction and improves the frontal and saggital walking balance. This child presented with severe torsional deformities and feet defomities. After correction his walking pattern has improved significantly. The brain 🧠 damage remain as it is and is not affected with any treatment.
