Fibroids are benign growths of muscle that occur in the uterus. They are very common and often appear during the reproductive years . Fibroids are described on the basis of their location inside the uterus
INTRAMURAL : They grow within the muscular wall of uterus and they are the most common type of fibroids .
SUBMUCOSAL : They bulge into the uterine cavity beneath the inner lining of uterus . These fibroids may sometimes be associated with cases of infertility
SUBSEROSAL: These fibroids grow beneath the serosa and project to the outside of the uterus .
CERVICAL FIBROID: They develop in the wall of cervix .
Fibroids are benign in nature and are not associated with increased risk of uterine cancer
CAUSES OF FIBROIDS: Exact cause is still unknown . Certain things increase the risk of developing fibroids .
• Genetic changes
• Hormones – Estrogen and progesterone promote the growth of fibroids
• Family history and Race – Black women and Africans are more prone to developed fibroids .
• Obesity
• Polycystic ovarian syndrome
• Diabetes
• Alcoholism
• Vitamin D Deficiency
• Eating red meat

SYMPTOMS OF UTERINE FIBROIDS : Many women develop uterine fibroids sometime in there life but may not be aware as it usually causes no symptom. Those who develop symptoms , include – heavy menstrual bleeding , frequent urination , pelvic pain including pain during s*x , backache , leg pains , difficulty in emptying the bladder , constipation

DIAGNOSIS OF FIBROID : They are usually diagnosed on routine pelvic examination or ultrasound getting done for some other cause . MRI may be done for location and size of fibroids

TREATMENT OF FIBROIDS : Medication may not remove the fibroid but may help in reducing the size of the fibroid
Non steroidal anti inflammatory drugs to relieve pain
Progestin releasing intrauterine device to reduce bleeding
Tranexamic acid during periods to control bleeding
Low dose of oral contraceptive pills

SURGICAL PROCEDURE may include:

Hystererectomy – removal of uterus by open surgery or laparoscopy . It is the only permanent solution to uterine fibroid

Myomectomy – removal of only fibroid

MRI guided focused ultrasound surgery ( FUS)
Uterine Artery Embolization – small particles are injected into the arteries supplying the uterus, cutting off blood supply to the fibroid , causing them to shrink
Endometrial ablation : A specialized instrument is inserted into the uterus to destroy the lining of uterus . It reduces menstrual blood flow and thus stops excessive bleeding .

Postmenopausal bleeding 16/12/2022

POST-MENOPAUSAL BLEEDING

When a woman stops to have periods for one year, the woman is said to have attained menopause. Any bleeding occurring after the period of menopause is called post-menopausal bleeding.
This bleeding can be in the form of a little spotting or bleeding like me**es.

If you have any such episode of post-menopausal bleeding, you must see a gynaecologist as soon as possible. It is of concern as 1 out of every 10 such patients suffers from uterine cancer. However, we must see the positive aspect that in 9 out of every 10 cases, the cause is usually benign; it is not at all associated with some cancerous condition. One must see a gynaecologist and get investigated. One must give a proper history regarding the use of any indigenous medicine; if the woman is taking hormone replacement therapy, if there is any history of trauma or any record of instrumentation, it should be informed to the doctor.
The doctor will do a local examination to see if there is any local cause of
bleeding. Next, a USG would be advised to see for the endometrial thickness and adnexa.
Endometrial thickness (ET) is the lining of the uterine cavity. If ET is more than 4mm, it is considered abnormal. Then a hysteroscopic guided biopsy would be advised. Hysteroscopically, an abnormal area is identified, and a biopsy is taken and sent for histopathology. The advantage of hysteroscopy is that if there is any polyp, it can be removed in the same setting.

90% of the time, the cause is usually benign. However, it can be atrophic vaginitis(60-90%), hormonal therapy(15-20%), endometrial hyperplasia, cervical or uterine polyp, endometrial cancer, or cervical cancer. After attaining menopause, due to hormonal deficiency, the walls of the va**na are thin, so any abrasion or scratch may lead to bleeding.
So any episode of postmenopausal bleeding should not be neglected.

Laparoscopic surgeries in Gynecology

How are laparoscopic surgeries performed?

Small access of 1cm are given on the abdomen and camera along with the instruments are introduced. By looking through the camera , the whole surgery is performed. Steps of the laparoscopic surgeries are same as that of open surgeries. Only the approach of performing the surgeries is different in laparoscopic and open surgeries.

Common types of laparoscopic surgeries performed in gynaecology:

Total Laparoscopic Hysterectomy with bilateral salpingectomy with or without oophorectomy- It involves the removal of uterus,tubes and ovaries

Myomectomy- removal of fibroid

Ovarian cystectomy or ovariotomy- removal of ovarian cyst or diseased o***y

Diagnostic laparoscopy in infertility patients-to visualize the pelvic organs and if any pathology found, it can be treated laparoscopically

Laparoscopic tubal ligation- as permanent method of contraception

Laparoscopic tubal recanalization- for opening up of previously ligated tubes

Advantages of Laparoscopic surgeries-

Since the operation is performed through tiny holes, patient has very little pain and discomfort after the surgery.

Recovery is faster.

Hospital stay is less; in most of the cases, patient can be discharged the same day or the next day.

Blood loss is less as compared to the open surgeries.

ABDOMINAL BELT AFTER DELIVERY / CAESAREAN SECTION

Wearing a postpartum belt or tying a cloth after delivery is very common in our country. Postpartum belt or binder can be worn after normal va**nal delivery as well as after caesarean section. After normal delivery, you may start wearing after 1or 2 days. After Cesarean section, you may wear it after 8-10 days when the caesarean site wound healing has occurred.

For what duration it should be worn?

Some people wear abdominal belts throughout the day, which is a wrong practice. It should be worn for 4-6 hours a day while sitting or doing various movements. It provides support to the abdominal muscles and the back.

Where should you tie the belt?

After normal va**nal delivery, it can be tied all over the area of lower abdomen. After Caesarean section, abdominal belt can be tied above the level of stitches

How much tight it should be?

Abdominal binder should have a moderate fit. If it is too tight, it would cause undue pressure to the involuting uterus and may cause excessive bleeding. If it is too loose,it may not serve the purpose.

Some people believe that wearing an abdominal belt may reduce the fat around the abdomen and it would help in bringing back the body to its original shape and size.

There is no scientific evidence to prove this. Wearing an abdominal binder helps only in providing support to the abdominal muscles and the back. To bring the body back into its original shape and size, one must follow the routine of doing abdominal exercises in the initial 2 or 3 months after delivery; increased protein intake in the diet and breastfeeding also helps to restore the body shape.

Certain side effects which may happen with the use of the belt-
• Edges of the binder may cause skin irritation which can cause skin rashes
• It may lead to sweating near the stitches and can result in wound infection
• If it is too tight, may cause excessive bleeding
