The deep caries on the distal along the impacted 8 was tricky.

Used a no 29 clamp from KSK for the last anchor since it has a deep bow extension that helps in vision and yet doesn't interfere with the clamp placement and airoter head for finishing.

The Haller clamp helped in further keeping the dam down and wide spread isolation.

Always remember to get the contacts and contours right.

Tips for esthetics.

It is difficult to pack a teflon into the sulcus most times because it tends to stick to the instrument and come out, difficult to roll it into a uniform mass.

Here my tip.

Use a piece of cord similar in length to the teflon and roll the teflon around it.

Easy peasy now.

You can easily pack the teflon in now and get double benefit of retraction with cord plus no gingival seepage.

Back to work

Putting up a case of back to back class II's. The patient had some pain while biting and no major discomfort according to her.

She has come for a checkup of another tooth but this was diagnosed on routine diagnostic x-rays and bitewings.

Crack Dam Creation on both the teeth and restoration using the Bioclear - biowhatever protocols.

Patient who is a dentist walks in with sensitivity on biting and cold. There was a previous amalgam restoration and a class I on the 7 both of which were failing.

We decided to do an onlay on the 6 and a direct restoration on the 7.

The steps in detail of the onlay design, prep and final bonding are shown below.

The MB2 was missed in the previous root canal. It was informed to the patient but he chose not to retreat it. A consent was taken.

What would you do in such a case? No periapical pathology, no symptoms, would you retreat? We choose to take a consent from the patient after informing him the pros and cons.

44TH ISPPD
Another one ticked off the box.

I happened to attend one of the ISPPD meetings when I was in BDS and i was always in awe of how things were organised with this particular body of Indian Dentistry and this year the team at Ahmedabad didn't disappoint either.

With the ever charming and enigmatic Dr. Rohan bhatt at the helm of things, ISPPD Ahmedabad was a spectacle. 24 pre conference courses in one day and none of the courses felt lifeless. The organisation was smooth and seemed effortless.

Thank you to the team for the invite and the lovely hospitality, although I couldn't stay back and attend the complete conference. I will take back a lot of fond memories.

1. You don't need two seperate rings for premolar and molar now. One would do.

2. Extreme seperating force - much needed with the thickness of the bioclear matrices.

3. The tines are much closer and hold on to areas very well. Better adaptation yet enough space for overmoulding.

4. The plastic is softer and hugs the tooth - even partially erupted ones or rotated ones - no more ring flying off in the operatory and slipping from the tooth.

5. Better space to even place a rubber dam ring on the same tooth. Attaching the pics below with a case where the ring and rubber dam ring are placed on the same rotated tooth.

Class III are sometimes the most notorious for they look small and easy but sometimes they can be a real challenge to match and finish.

A case of emax onlay.

I had done the class II to this patient about 6 years back. He bit on something hard and fractured a cusp almost 6 months ago. Now the restoration got dislodged and he came because he had sensitivity.

The most conservative restorations in such cases ia always an onlay.
