April is head and neck cancer awareness month

Signs and symptoms of oral cancer which is predominantly caused by to***co usage and/or excessive alcohol usage may include one or more of the following:

1) Any sore or ulceration that does not heal within 3 weeks
2) A red, white, or black discoloration of the soft tissues of the mouth.
3) Any abnormality in the mouth that bleeds easily when touched (friable).
4) A lump or hard spot in the tissue, usually at the border of the tongue (induration).
5) A sore under a denture, which even after adjustment of the denture, does not heal.
6) A lump or thickening that develops in the mouth.
7) A painless, firm, fixated lump felt on the the neck, which has been there for at least 3 weeks.

All the above symptoms have one common thing is that they are persistent and non- resolving.

Axillary management of breast cancer has changed tremendously over the past few decades.

The use of sentinel node biopsy over ALND has the benefit of reduction in morbidity, while not compromising staging, survival or locoregional recurrence.

The increasing effectiveness of adjuvant systemic and local therapies has been key to these advances in surgical care.

BCS (Breast Conservation Surgery) case diary:

A 40yr old lady with 4x3x2.5cm left breast lump at 12 o clock position, being diagnosed as Ca breast on core biopsy. I did BCS (Batwing mastopexy) with intraop frozen section for confirmation of margins, free of tumor alongwith axillary lymph node clearance. I was able to give good cosmetic outcome to the patient alongwith oncological clearance.

MRM (complete removal of breast) is not the only treatment to breast cancer. BCS can also give similar long-term survival outcome as MRM provided it is done in a technically correct method and keeping into the mind the criteria of tumor fitting into BCS category.

Breast conservation surgery for multicentric breast cancer: is it really possible??

Multicentric breast cancer is 2 or more tumors in separate quadrants of the breast that cannot be included in a single excision.
This is currently an absolute contraindication to breast conservation surgery.

The Alliance Z11102 trial showed that multiple cancers in the same breast that are >2cm apart can be removed with breast conservation surgery in 93% of patients ( majority of them reported excellent cosmesis).
If locoregional recurrence data of the trial are favorable,then this could be a big stride in breast conservation surgery.
