This patient came to us with difficulty walking, slowness in activities and hand tremors.
He couldn’t visit his orchard farms since last few months due to his condition and daily activities were becoming difficult day by day.
Detailed evaluation of his symptoms suggested Parkinson’s disease and started him on medication and neuro rehabilitation program.
This was his 2nd visit in which he was happy to share his story with us and many more like him.

Dystonia Part -2

*Treatment of focal dystonias*
Botulinum Toxin injection is the treatment of choice for focal dystonia including blepharospasm, cervical dystonia, writer’s or musician’s cramp.
Botulinum toxin reduced the overactivity of dystonic muscles temporarily and helps in reducing the dystonic symptoms.

It is important to target the selected dystonic muscles for injection, otherwise the results will not be satisfactory or patient can have side effects if injected in normal muscles.
Therefore EMG guided botulinum toxin injections gives the best results in which muscles can be targeted by checking their actual activity and injecting the medicine at the same time.

The ON and OFF effect of Levodopa - when a person with Parkinson’s disease takes levodopa - he or she may feel better after 30 or 40 minutes in which symptoms resolve or are better - less tremors, less stiffness and less slowness and better walking.
After few hours - 3 to 4 hours or depending on patient to patient - the medication effect wears OFF - in which symptoms recur - increased tremors, stiffness and slowness and difficulty in walking with loss of balance.
It’s important to understand the ON and OFF effect so that your doctor can manage your medications well.
So do note your ON and OFF time for better management of your Parkinson’s symptoms.
