𝑪𝒉𝒂𝒍𝒍𝒆𝒏𝒈𝒊𝒏𝒈 𝑪𝒂𝒔𝒆
The patient, an endodontist herself, reported a history of initiating RCT in the right upper second molar, which was followed by the placement of a restoration few years back. Recent pain in the tooth prompted further investigation. Initial intraoral periapical radiographs (IOPA) revealed a restoration and a minute radio-opaque object within the pulp chamber. On removal of the restoration and microscopic exploration, it was discovered that the radio-opaque object was a separated file positioned at the entrance of the MB canal. The skilled use of ultrasonics facilitated the successful retrieval of the separated instrument. After chemo mechanical preparation, the patient experienced pain and bleeding in the palatal canal, and no definitive apical stop was achieved in both the MB and DB roots. The CBCT revealed an accessory palatal canal, and both the MB and DB roots exhibited signs of resorption resulting in significantly shortened lengths. With the aid of advanced magnification, the accessory palatal canal was precisely located and prepared. Subsequently final obturation was done.

𝑴𝒂𝒙𝒊𝒍𝒍𝒂𝒓𝒚 𝒄𝒂𝒏𝒊𝒏𝒆𝒔 𝒘𝒊𝒕𝒉 2 𝒄𝒂𝒏𝒂𝒍𝒔: Maxillary canines are statistically considered to be single-rooted, single-canaled teeth and two root canals in a permanent maxillary canine is a rare condition. In this case there were 2 canals with 2 separate orifices in maxillary canine.

𝑺𝒉𝒐𝒕 𝒖𝒏𝒅𝒆𝒓 𝑴𝒊𝒄𝒓𝒐𝒔𝒄𝒐𝒑𝒆

𝐋𝐨𝐜𝐚𝐭𝐢𝐧𝐠 𝐌𝐁𝟐: This patient came with the chief complaint of pain on consuming Hot things. Got his root canal started from some other clinic. After multiple appointments with previous dentist, he still had pain. After opening the chamber MB1, DB and Palatal were already prepared. The only reason for pain was missed MB2 canal. After troughing with ultrasonics under microscope located the MB2 and obturated the canals.
In Upper molars missed MB2 is the main cause of pain and root canal failure most of the times.
